A
A scheme aimed at providing referrals to activities/ support groups where medical interventions alone may not adequate
Range of local, non-clinical services supported by a link-worker or connector

History mental health problems
Frequent GP attendees
2+ long term conditions
Socially isolated
Untreatable/ poorly understood LT conditions (IBS, fibromyalgia, chronic fatigue)
Those not benefiting from clinical/ drug treatment
