Amiodarone

A

V-Fib, P.V-Tach:
300mg
Repeat 1x: 150mg after 3-5min

Unstable V-Tach w/pulse:
150mg IV (mix w/ 100mL NS, infuse over 10min, WaCounty) bolus over 10min @ 15mg/min, q 10min. 

V-Fib; P.V-Tach (non responsive to CPR, shock, or vasopressors)
Unstable V-Tach w/pulse (when Cardioversion ineffective.

A

1mg/kg ( ACLS)

Lower dose for Vent Ectopics if: Pt > 70y/o, Shock, Liver Failure
1mg/kg q 10min w/ 0.5mg/kg, max 2mg/kg.

Pain Mngmt for IO- not to exceed 50mg

VFib/P.VT, VT w/pulse, PVC’s (V ectopics)

A

15-20mg (0.25-0.30mg/kg) sIVP (1-2min)
20-25mg (0.30-0.35mg/kg) “ q 15min.

RVR w/: A-Fib, A-Flutter, Angina,
After Adenosine for P.SVT w/ narrow QRS & stable BP.

Magnesium Sulfate

A

VF/VT: 2g over 1-2mins. Dilute each gram (2mL) of Mag in 8mL NS. Example: mix 1g (2cc) in 8 cc’s NS; mix 2g (4cc) in 16 cc NS.

Per AHA: Torsades or suspected hypomagnesia
VF/VT: Cardiac Arrest after Defib, Epi, Lido or Ami.
