Skin Rash Flashcards
A 9-year-old child presents with crusting lesions on the face and a mild fever. On examination, there are a number of honey-colored crusting lesions in the nasolabial folds of the face. Crust culture revealed Gram+ catalase negative beta-hemolytic cocci.
Impetigo
A 67-year-old fisherman presents with a non-healing ulcer for the past 3 months. On examination, there is a 1 cm wide ulcer on the side of his nose. Biopsy reveals neoplastic basal cells.
Basal cell carcinoma
A 2-year-old infant presents with an itching rash on his face and arms. There is a history of hay fever and asthma in the family. On examination, there is a red, oozing rash on the cheeks and flexural aspects of the elbows and knees. No other tests done.
Eczema
An 18-year-old teenager presents with an itching rash in the groin. On examination, the rash has a raised erythematous serpingous advancing border and a healing center. No other tests done.
Tinea corporis
A 19-year-old teen presents with a rash on the forearm with fever and fatigue. On examination, there is a targetoid rash on the forearm. ELISA for B. burgdorferi organism is positive.
Lyme’s disease
A 23-year-old female presents with a strange looking mole on her shoulder. On examination, the lesion has irregular borders and is very dark in color. Biopsy showed malignant melanocytes.
Malignant melanoma
A 5-year-old unimmunized child present with a high fever and a runny nose followed by a rash that started on the trunk and is spreading. On examination, there are Koplik spots on the inside of the cheeks and a fine maculopapular erythematous rash on the trunk and extremities. No other tests were done.
Measles
A 35-year-old overweight female presents with a skin rash. On examination, there are a number of silvery plaques on the back of her elbows and scalp. No other tests were done.
Psoriasis
A 3-year-old child presents with a rash on both cheeks following a fever. On examination, there is a red rash on both cheeks as if someone slapped the child. No other tests were done.
Slapped cheek syndrome (aka Fifth disease)
A 15-year-old teen presents with numerous pimples and blackheads on their face. On examination, there are many comedones and pustules with some scarring on the face and upper back. No other tests done.
Acne vulgaris
A 19-year-old black female presents with a rash on her face and joint pain. Her hands and right knee have been painful for the past 3 weeks. On examination, there is a malar rash with swollen PIP, MCP and knee joints. ANA and anti-ds-DNA antibodies are present.
SLE
A 25-year-old male presents with finger tips that were tight and painful. On examination, there is bluish discoloration of the hands and shiny, tight skin over his fingers. He also has crepitus related to the tendons in his forearm. ANA and SCL-70 antibodies are present.
Scleroderma
A 73-year-old male presents with a non-healing lesion on the cheek for the past six months. On examination, the rash is a red, scaly and raised plaque. There are no enlarged regional lymph nodes. Biopsy reveals neoplastic keratinocytes.
