lesions
factors to inspect in nails
shape and contour
consistency
color
capillary refill (return of color after pressing)
ABCDEF skin assessment
A : asymmetry B : border C : color D : diameter E : elevation and enlargement F : funny looking - different from others
striae (gravidarum)
stretch marks
linea nigra
dark line down midline
chloasma
hyperpigmentation on face
senile lentigines
liver spots
senile purpura
capillary fragility causing discoloration
annular
circular pattern, begins in middle and spreads in periphery
confluent
lesions run together (hives)
discrete
distinct individual lesions (acne)
gyrate
swirled, twisted, coiled
grouped
cluster arrangement (contact dermititis)
linear
in a line, like a scratch
target
bullseye
zosteriform
linear along single nerve route, only on one side
polycyclic
annular but together (psoriasis)
macules
freckles, flat and circular, under 1 cm
papules
moles/warts, elevated, under 1 cm
nodules
solid, elevated, larger than 1 cm
wheal
raised mosquito bite
vesicle
fluid filled pocket - pox/blister, less than 1 cm
bulla
fluid filled pocket - pox/blister, greater than 1 cm
cyst
fluid filled cavity
pustule
circular, elevated, filled with exudate (acne)
crust
thick, dried up exudate (from pustule or vesicle that burst)
scale
flakes of dried skin
fissure
linear crack, abrupt edges (athlete’s foot)
erosion
moist, shallow
ulcer
deeper depression, like bed sores on pressure points
excoriation
abrasion from scratching
scar
tissue replaced with collagen after injury
atrophic scar
epidermis has thinned out at point of injury healing
lichenification
thickened skin due to prolonged intense scratching
keloid
benign excess of scar tissue (extends beyond skin surface)
petechiae
bleeding from capillaries (tiny) can indicate clotting problem
purpura
extensive petechiae, can lead to/include bruising
ecchymosis
purplish patch from blood leaking into skin (bruise)
primary contact dermatitis
local inflammatory reaction to irritant or allergy
tinea corporis
scales with pigmentation opposite of regular skin color, multiple annular lesions with clear centers
AIDS-related Kaposi sarcoma
vascular cancer in HIV pt. presents in patch-stage as faint pink lesions
clubbing of nails
nail bed angle greater than 180*, makes distal phalanx look rounder, wider and shiny. (can be from chronic lung inflammation, lung cancer, heart defects with r-to-l shunts)
