Resembles of eye, concentric rings of color in the lesion
Ex: Erythema multiforme
Target/iris
Scratch, streak, line or stripe
Linear
Annular lesions that grow together
Ex: Lichen planus, psoriasis
Polycyclic
Linear arrangement along a nerve route
Ex: Herpes zoster
Zosteriform
Soley a color change
Flat and circumscribed
< 1cm diameter
EX: Freckles, flat nevi, hypopigmentation, petechiae, measles, scarlet fever
Macule
Macules > 1 cm diameter
EX: Mongolian spot, vitiligo, cafe au lait spot, chloasma, measles rash
Patch
Palpable, solid, elevated, circumscribed, < 1 cm diameter
Caused by superficial thickening in the epidermis
EX:
Elevated nevus (mole), lichen planus, molluscum, wart (verruca)
Papule
Papules that coalesce to form elevation wider than 1cm
Plateau like, disc shaped lesion
Ex:
Psoriasis, lichen planus
Plaque
Superficial, raised, transient, and erythematous, slightly irregular shape because edema (fluid held diffusely in the tissues)
Ex: Mosquito bite, allergic reaction, dermographism
Wheal
Wheals that coalesce to form extensive reaction; intensely pruritic
Urticaria/Hives
