Eczema/Atopic Dermatitis
Sx: Hx of allergies, Red weeping crusted lesions, mb lichenification
PE: All the obvious ones
Labs: Skin scraping/bx
Ddx: Seborrheic, Contact, Nummular derm, Candida
Tx: Prednisone, Allergie free diet, EFA, Vit A & E lotion
Where is Eczema more commonly found in children?
Face, neck, extremities, abdomen
Where is Eczema more commonly found in adults?
Flexor surfaces of elbow and knee, buttocks, neck, hands
Contact Derm
Sx: Ranges from erythema to bull, vesicles and pruritus common. ICD is painful, ACD is pruritic.
Labs: Skin patching for allergies
Ddx: Atopic derm, Herpes simplex, Herpes Zoster
Tx: Allergie free diet, Oatmeal baths, Aloe vera, Calendula
Tineas….all of them
Tinea corporis: Sx-Red papulosquamous, annular lesions. Ddx-Pityriasis rosea, erythema multiforme, nummular Derm
Tinea pedis: Sx- Painful vesicles, mb secondary to bacterial infx. Ddx-Contact derm
Tinea capitis: Black dots from broken hairs. Ddx-psoriasis, seborrheic derm
Tinea unguium: Infx of the nail, grows thick and dull. Ddx-psoriasis.
PE: Woods Lamp
Lab: Skin scraping
Tx: Topical Lotrimin, tea tree oil, probiotics, EFA
Psoriasis
Sx: Dry silvery scale w erythematous base rash located on extensor surfaces, scalp, buttock, back. Pruritus. Arthritis (1 sausage digit).
PE: + Auspitz sign, Assess DIP, PIPs.
Ddx: Seborrheic, SCC in situ (when on trunk), Lichen plants, Dermatitis
Tx: Methotextrate, Vit D creams, GF
Impetigo
Sx: Cluster of vesicles/pustules that rupture w honey colored crusts. Pruritic, located on the face, regional LA
Labs: Culture vesicles
Ddx: Atopic, Contact derm, if group A strep ro glomerulonephritis
Tx: Penicillin, Tea tree oil, Zn
Urticaria
Sx: Pruritis first then wheals, may have angioedema/edema of upper airway.
Labs: CBC, UA, LFT, Allergy testing, IgE serum
Ddx: Drug eruption, Contact derm, Erythema multiform, SLE/Lymphoma if >3w
Tx: Allergens, Nettles, Vit C
