Digoxin
A
- Trade: Lanoxin
- Family: Anti-arrhytmic
- Therapeutic effect:
- Increase strength of Heart contractions,
- Control Rate and Rhythm, (Pts with CHF, Afib, Aflutter)
- Side effects:
- CAREFUL ADMIN,
- Dizziness,
- N/V/D,
- Visual disturbances
- Nursing issues:
- Check APICAL HR - Hold if <60,
- Monitor for toxicity (N/V/D, vision issues),
- Narrow Therapeutic range
- Patient teaching:
- Call if HR <60 or >110,
- Report weight gain of >2lb,
- No OTC without approval

Furosemide
A
- Trade: Lasix
- Family: Loop Diuretic
- Therapeutic effect:
- Lower fluid retention,
- Inhibit reabsorb of Na & Cl
- (pts w/edema, liver or kidney disease)
- Side effects:
- Tinnitus,
- Jaudice,
- Severe stomach pain (N/V/D)
- Nursing issues:
- BP and vital,
- S&S hypkalemia,
- I&O dehydration
- Check K and electrolyte levels,
- SJSyndrome,
- Anemia
- Patient teaching:
- dizziness,
- high K diet,
- BGL - can rise,
- Don’t take at night to avoid urinating at night

Heparin
A
- Family: Anticoagulent
- Therapeutic effect:
- Prevent blood clots
- Prophylaxis and treatment for DVT and PE
- Side effects:
- Easy bleed and bruise,
- Pain,
- erythema,
- irritation
- Nursing issues:
- Labs on Hct, Hgb, RBC, platelets
- Coag draw 30 mins b/f SC/IV dose
- Monitor - fever, Drop in BP, Increased HR, other signs of hemorrhage
- Patient teaching:
- Call HCP if urine not yellow,
- Menstruation may be long,
- No smoke or alcohol

- Generic: Humulin N, L, Novoliln, NPH (intermediate acting, cloudy)
- Family: Antidiabetic, Pancreatics
- Therapeutic effect: Low BG, Control BG
- Side effects:
- Drop in BG
- Headache
- sweating
- irritability
- Nursing issues:
- monitor BG depending on type
- Call HCP if acetone and sugar in urine,
- Monitor for hypoglycemia,
- check IO hourly with IV
- Patient teaching:
- Injection technique,
- Inject in abdomen,
- hypoglycemia response,
- carry fast acting carbs,
- notify HCP of diabetic ketoacidosis

- Family: Miotic (cholinergic)
- Therapeutic effect: Reduce ocular hypertension (Glaucoma)
- Side effects:
- Increase Sweating,
- N/V/D
- Nursing issues:
- Drops in conjunctiva,
- Monitor Interocular pressure with tonometric test,
- Monitor pain and vision changes
- Patient teaching:
- Must adhere - prevent blindness,
- Discontinue if worsening

Nexterone
A
- Generic: Amiodarone
- Family: Anti-arrhythmic
- Therapeutic effect: Restore normal heart rhythm (Vfib, Vtac, Prophylaxis for heart rhythm disorders)
- Side effects:
- Lower effects of Digoxin,
- Check eyes - lens opacity/spots,
- Respiratory distress,
- Depressed HR/BP,
- N/V/C,
- Photosensitivity,
- Syncope in Bradycardia,
- Not in iodine allergy,
- Liver fx
- Nursing issues:
- Caution in Digoxin pts (reduced dose),
- Caution in Warfarin (reduce dose),
- Patient teaching:
- No grapefruit juice or St. Johns Wort,
- Monitor pulse daily - report HR <60,
- Wear sunscreen,
- Take with food

Aspirin
A
- Generic: Family: Analgesic (nonopiod), antipyretic
- Therapeutic effect:
- Blood thinner - prevents clots,
- Lower Pain and Fever and headache and MI risk
- Side effects:
- Stomach Pain,
- Tinnitus
- Nursing issues:
- Ask if on any other blood thinners,
- Caution w/platelet and bleeding disorders,
- Avoid 1-2 weeks b/f surgery (excessive bleeding)
- Patient teaching:
- Signs of allergic reaction,
- Signs of bleeding (coffee ground vomit, tarry stools, bleeding gums, bruising)

Metoprolol
A
- Trade: Lopressor
- Family: Beta blocker
- Therapeutic effect: Lower BP and Reduce angina (Pts with Hyper tension and angina)
- Side effects:
- Drowsiness,
- Slow HR,
- Heartburn
- Nursing issues:
- BP/HR and rhythm (BP <60BPM or SBP <90 hold and call),
- High BP and CHF (worsening),
- CNS impact - depression,
- Diabetes - monitor for glycemic spikes
- Patient teaching:
- Sit after dose,
- No abrupt withdrawal,
- Check HR/BP - HR<60 call,
- no smoking,
- Report vision trouble,
- Can give pts nightmares

- Family: Analgesic (nonopiod), antipyretic
- Therapeutic effect: Reduce pain and fever (minor aches and pains)
- Side effects:
- HEPATOTOXIC,
- N/V,
- Stomach pain,
- Headache
- Nursing issues:
- Check liver fx labs,
- MAX dose 3g per day,
- Aspirin allergy,
- hemostatic disturbances,
- bleeding,
- Upper GI disease,
- Gout-arthritis,
- Arthritis and rheumatic disorders involving musculoskeletal pain
- Patient teaching:
- DO NOT EXCEED recommended dose (no longer than 10 days),
- Not an anti-inflammatory,
- DO NOT take with other acetaminophen containing drugs

A
- Gerneric: Naproxen
- Family: Analgesic (nonopiod), antipyretic NSAID
- Therapeutic effect: Reduce fever, pain AND inflammation
- Side effects:
- GI,
- Dizziness,
- Headache
- Nursing issues:
- Caution - low Hgb, kidney or liver fx,
- Caution - allergic to NSAIDS/aspirin,
- Prolongs bleeding time
- Patient teaching:
- Effects may take 3-4wk,
- Avoid alcohol and aspirin while taking

- Trade: ISMO
- Family: Nitrate
- Therapeutic effect:
- Prevent angina,
- Manage long term angina pectoris (Pts with angina, coronary artery disease),
- Lower BP - Up Blood flow
- Side effects:
- Headache,
- N/V/C/D,
- Dizziness/lightheaded
- Nursing issues:
- Orthostatic hypotension,
- Monitor pain relief,
- Use analgesic to treat headaches,
- Monitor BP/HR
- Patient teaching:
- Move slowly - lightheadedness,
- No Alcohol (dizziness),
- Headache common - treat with analgesic - do not change dose,
- Take last dose of day before 7pm,
- Call if blurred vision or dry moutn

- Generic: Diliazem
- Family: Calcium channel blocker, (Antianginal, antiarrhythmic, antihypertensive)
- Therapeutic effect: Lower HR (Afib, Aflutter, Supraventricular tachycardia, Chest pain)
- Side effects:
- Headache,
- N/V/D/C,
- Angina,
- SJSyndrome (skin peel),
- Nursing issues:
- Check BP and ECG and monitor throughout,
- HOLD IF MAP <65,
- Monitor Liver and renal fx,
- Monitor for s&S of CHF
- Patient teaching:
- Sit after taking,
- Move slowly (orthostatic hypotension),
- No driving,
- Monitor pulse (under 50 call),
- no grapefruit juice,
- sunscreen

A
- Trade: Colace
- Family: Stool softener
- Therapeutic effect: Lower surface tension in bowel, Prophylactic for pts to avoid straining while defecating
- Side effects: N/D, Abdominal cramps
- Nursing issues:
- W/H if diarrhea - notify HCP,
- Effect takes 1-3 days
- Patient teaching:
- Take with liquid and increase liquid in take,
- No mineral oil,
- Only for prescribed time period - monitor diet
