- Knock and introduce yourself
- Perform hand hygiene
- Check to make sure that is the correct resident
- Explain the procedure to the resident
- Provide privacy (pull curtian)
- Make sure the bed wheels are locked
- Place a barrier before putting any materials down
Explain how to perform hand hygiene.
- Cover all surfaces of hands with hand sanitizer
- Rub hands together until hands are completely dry
Explain how to perform hand washing.
- Wet hands and wrists thoroughly and apply soap to hands
- Rub hands together using friction with soap
- Scrub/wash hands together for at least twenty (20) seconds with soap
- Scrub/wash with interlace fingers pointing downward with soap
- Wash all surfaces of hands and wrist with soap
- Clean fingernails by rubbing fingertips against palm of the opposite hand
- Rinse fingers, hands and wrists thoroughly with fingers pointed downward
- Dry fingers, hands and wrists on clean paper
towels - Discard paper towels to trash container as used
- Turn off faucet with a clean, dry paper towel and discard paper towel
- Do not re-contaminate hands at any time during the hand washing procedure

- Gently remove bedpan
- Hold bedpan for RN Tester to fill with liquid
- Place graduate on a flat surface and fill with liquid
- Mesure output in ml’s
- Empty liquid into toliet
- Rinse bedpan and graduate and empty water into toliet
- Complelety dry both and store away
- Assist resident to wash and dry hands with soap and water using 3 wash clothes
- Place dirty wash clothes in laundry hamper
- Remove gloves properly and dispose them
- Record output in ml’s on the reporting form
- Candidate’s recorded measurement is within 25ml’s of RN Test Observer’s reading

- Fill basin with comfortably warm water

- Expose area surrounding catheter only
- Hold catheter where it exits the urethra with one hand
- Use a clean portion of the washcloth for each stroke with soap
- Clean with strokes only away from the urethra 3 inches down
- Rinse using strokes only away from the urethra
- Rinse using a clean portion of the washcloth for each stroke
- Pat dry
- Replace top cover over resident
- Remove and dispose barrier and place dirty linen in laundry hamper
- Empty, rinse, dry and return basin
- Properly remove gloves

Explain how to empty a urinary drainage bag.
- Preform before care

- Open drainage bag and completely empty
- Avoid touching the graduate with the tip of the tubing
- Close the drain
- Wipe the drain with an alcohol wipe AFTER emptying drainage bag
- Place graduate on a level flat surface and measure output
- Empty graduate into toliet
- Rinse, dry, and store graduate
- Dispose barrier
- Record the output in ml”s on reporting form
- Candidate’s recorded measurement is within 25ml’s of the RN Test Observer’s measurement

- Apply lotion to the foot except in between the toes

- Bend the resident’s knee and hip toward the resident’s trunk (flexion)
- Straighten the resident’s knee and hip (extension)
- Complete flexion and extension of the knee and hip at least three times
- Must ask resident at least one time if they feel any pain or discomfort

- Complete flexion and extension of the shoulder at least three times
- Move the resident’s entire arm away from the side of the resident’s body to shoulder level (abduction)
- Return resident’s arm to the side of the resident’s body (adduction)
- Complete abduction and adduction of the shoulder at least three times
- Must ask resident at least one time if they feel any pain or discomfort

- Count resident’s respirations for one full minute (tell RN Tester when you start and stop counting)
- Record the resident’s respirations reading on the recording form
- Respiratory rate must be within 2 breaths of the RN Tester
