3 Assists to Ambulate Using Transfer Belt

A

•Evaluator will tell the destination

  1. Explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible

Before assisting to stand the following steps must be done: (*may be done in any order)

  1. *“Bed is at a safe level” for client to get up
  2. *Checks and/or locks bed wheels
  3. *Client is assisted to a sitting position with feel flat on the floor
  4. *Client is wearing shoes
  5. *Puts on gait belt securely at waist over clothing
  6. Provides instructions to enable client to assist in standing including prearranged signal to alert client to standing
  7. Stands facing client positioning self to ensure safety of candidate and client during transfer
  8. Counts to three (or prearranged signal) to alert client to begin standing
  9. On signal, gradually assist client to stand by grasping transfer belt on both sides with an upward grasp (candidates hands are in an upward position)
  10. On signal, gradually assist client to stand by maintaining stability of clients legs (your knee touching their knee)
  11. Walks slightly behind and to one side of client for a distance of (10) feet while holding onto the belt. (fingers, hand pointing up)

After ambulation:

  1. Assists client to bed (okay to leaving sitting)
  2. Removes transfer belt
  3. Signaling device is within reach
  4. Bed is in low position
  5. After completing skill, wash his hands

*If your patient is tall you may need to raise the bed. Remember to lower it when the pt is back on bed

  7. Places bedpan in designated dirty supply area

8 Donning and Removing PPE (gown & gloves)

• Supplies at the PPE Station

  1. Picks up gown
  2. Unfold gown
  3. Facing the back opening of the gown places arms through each sleeve
  4. Fastens the neck opening
  5. Secures gown at waist making sure the back of the clothing is covered by gown (as much as possible)
  6. Puts on gloves
  7. Cuffs of gloves overlap cuffs of down
  8. With one gloved hand, grasps the other glove at the palm
  9. Then remove glove (ball up in gloved hand)
  10. Slips fingers from ungloved hand
  11. Removes glove turning it inside out as it is removed
  12. Disposes of gloves into designated waste container without contaminating self
  13. Unfastens down at neck
  14. Unfastens down at waist
  15. Removes gown without touching outside of gown

While removing gown

  1. Hold gown away from body without touching floor
  2. Turns gown inward

  10. Candidate inflates cuff between 160mm Hg to 180mm Hg
  11. Deflates cuff slowly and notes the first sound (systolic reading) and last sound (diastolic reading)(If rounding needed, measurements are rounded up to the nearest 2mm Hg)

  14. Records both systolic and diastolic pressures each within plus or minus 8mm of evaluators reading

Note: * If beat heard immediately upon cuff deflation, completely deflated cuff. Reinflate cuff to 200mm Hg, they will use this as their systolic number and continue to listen for the diastolic number and a record. If they go over 200 on any attempt they will be given a SAFETY STOP and FAIL

  2. Applies soap to wet washcloth
  3. Washes genital area, moving from front to back
  4. While using a clean area of the washcloth for each stroke (1 stroke okay)
  5. Using clean washcloth, rinses soap from genital area, (1 stroke okay)
  6. Moving from front to back
  7. While using a clean area of the washcloth for each stroke (1 stroke okay)
  8. Dries genital area from front to back with towel

After washing genital area

  1. Turns manikin to side

22 Transfers from Bed to Wheelchair Using Transfer Belt

A
  1. Explains procedure, speaking clearly, slowly, and directly, maintain face-to-face contact
  2. Privacy is provided with a curtain, screen, or door

Before assisting to stand (*all must be done before client is standing)

    • Wheelchair is positioned a long side of bed, at head of bed, facing foot of bed or at foot of bed facing the head of bed
    • Footrests are folded up or removed
    • Locks wheels on wheelchair
    • “Bed is at safe level” for client to stand
    • Locks bed wheels (push on bed)
    • Client is assisted (hands-on) to sitting position with feet flat on floor
    • Client is wearing shoes
    • Applies transfer belt securely at waist over clothing/down
    • Provides instruction to enable client to assist in transfer including prearranged signal to alert when to begin standing
  1. Stands facing client positioning self to ensure safety of candidate and client during transfer
  2. Knee to knee, counts 1, 2, 3 to alert client to begin standing
  3. Gradually assist client to stand, grasping transfer belt on both sides with an upward grasp
  4. Maintaining stability of clients legs (knee touching)
  5. Assists client to turn to stand in front of wheelchair
  6. The back of the clients legs are touching the seat of the wheelchair
  7. Lowers client into wheelchair
  8. Positions client with “hips touching back of wheelchair”
  9. Transfer belt is removed
  10. Positions feet on foot rests
  11. Signaling device is within reach
  12. After completing skill, washes hands
