Applying an anti-embolic stocking on one of residents legs
Things needed: Sock
Key step: the stocking smooth, no pressure on the toes
Assisting a resident to ambulate using a gait belt
Things needed: Gait belt, a wheelchair, nonskid footwear (assistive devices if needed)
Key steps: Locks on bed are locked and Lock wheelchair
Assisting resident who is dependent with a meal
Things needed: Food with tray, clothing protector, hand sanitizer(/ soapy and wet cloths) diet card and eating utensils and gloves
Key Steps: Calculate food eaten and liquid intake
Assisting a resident with a bedpan with a hand washing required
Things needed: Gloves, two washcloths, a towel, toilet paper, bedpan and towel for barrier (might need linen bag)
Key steps: locks on bed are locked, make sure bedpan is placed correctly and liquid and rinse of bed pan goes into TOILET, wash hands
Catheter care for female resident
Things needed: Barrier, Basin with warm water, two washcloths, towel, gloves, soap and linen bag
Key steps: Hold catheter to make sure it doesn’t get pulled, check for flow(verbalize this step as well), clean with strokes away 3-4 inches, wash hands
Denture care
Things needed: Barrier, emesis basin, denture cleanser, brush, dentures, paper towel for sink, gloves
Key Step: make sure there is a lining for sink, make sure to do this with cool water
Isolation gown + empty urinary bag
Things needed: Gown, gloves, graduated cylinder, alcohol wipe, two barriers
Key steps: Back of gown is covered and tied, gloves are over the gown, don’t let catheter touch the edge of jar, record measurements, wash hands
Dressing with a weak side
Things needed: Clothes and linen bag
Key steps: Dress the weak side (DWF) first , undress the strong side first (USF)
Foot care on one foot
Things needed: Basin with warm water, gloves, soap, lotion, towel, 2 washcloths
Key steps: Dry between toes, do not get lotion between toes, give a small massage
Modified bed bath
Things needed: Barrier, Bed bath blanket, gown, Basin with warm water, 2 washcloths, towel, soap, lotion, linen bag
Keep things in mind: do not use soap in the face, change the gown
Mouth Care
Things needed: barrier(paper towel), emesis basin, tooth brush, toothpaste, cup of water, clothing protector
Key steps: verbalize, upper and lower, inner chewing and outer parts and brush tongue
Perineal Care on female
Things needed: Barrier, basin with warm water, Soap, 4 wash clothes, towel, gloves and linen bag
Key steps: Let test observer know to move on the opposite side of the bed and can help hold manikin (Infront) wipe from the top down (buttocks) wipe from vagina to rectal area, wash hands
Positioning patient in bed on their side
Things needed: three pillows
Key steps: have test observer stand on the opposite side of the patient that will be moved to, or raise side rail
Range of motion for residents hip and knee
Key steps: HOB will be elevated, make bed flat, must ask patient is there is any discomfort/pain
Range of motion for residents shoulder
Hey steps: HOB will be elevated make bed flat, must ask patient if there is any discomfort/pain
Stand and pivot transfer from bed to wheelchair
Things needed, gait belt, wheelchair
Key steps: Lock bed!, Lock wheel chair, assist client to pivot in a controlled manner
Vital Signs
Things needed: a hand watch
Key steps: Let test observer know when to enter the room, let the test observer when to begin and stop when starting pulse and respiration.
