SKILL 1: Hand washing
1.Sanitize/ wash hands and knock on the door
- Address client by name and introduce yourself
3.Turn on water
4.Wet hands and wrists thoroughly
5.Apply soap to hands
6.LATHERS ALL SURFACES OF WRISTS, HANDS, AND FINGERS PRODUCING FRICTION FOR ATLEAST 20 SECONDS, KEEPING HANDS LOWER THAN THE ELBOWS AND THE FINGERTIPS DOWN
- Cleans fingernails by rubbing fingertips against palms of opposite hand
8.RINSE ALL SURFACES OF WRISTS, HANDS, AND FINGERS, KEEPING HANDS LOWER THAN THE ELBOWS AND THE FINGERTIPS DOWN
- Uses clean dry paper towel(s) to turn off faucet then disposes of paper towel(s) into waste container
- Use paper towel to turn off faucet
- Don’t touch inside of sink at any time
SKILL 2: Applies one knee-high elastic stocking
- Sanitize/wash hands and knock before entering
- Introduce yourself and check patients wristband
- Explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible
- Provide privacy by closing the curtain
- Raise the bed to a comfortable position and lock the bed
- Client is in supine position (laying flat) while stocking is applied
- Turn stocking inside-out, atleast to the heal
- Pulls top of stocking over foot, heel, and leg
- Moves foot and leg gently and naturally, avoiding force and over-extension of limb and joints
- FINISHES PROCEDURE WITH NO TWISTS/WRINKLES, AND THE HEEL OF THE STOCKING IS OVER THE HEEL AND OPENING IN TOE AREA.
- Put bed back to low position and make sure call light button is in reach
- Open curtain, wash hands, record and report
SKILL 3: Assist to ambulate using transfer belt
- Sanitize/wash hands and knock before entering
- Introduce yourself and check patients wristband
- Explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible
4.Provide privacy with curtain
- BEFORE ASSISTING TO STAND, CLIENT IS WEARING NON-SKID SHOES/FOOTWEAR
- Before assisting to stand, bed is at a safe level
- Before assisting to stand, make sure the bed is locked
8.BEFORE ASSISTING TO STAND, CLIENT IS ASSISTED TO SITTING POSITION WITH FEET FLAT ON THE FLOOR
- Before assisting to stand, put transfer belt OVER clients clothes and making sure to put TWO fingers between the belt and patient
- Before assisting to stand, provide instructions for patient before they stand on the count of 3
- Stand facing client positioning self to ensure safety of candidate and client during transfer. Count to 3 to tell patient to begin standing
- On signal, gradually assist client to stand by grasping transfer belt on both sides with an upward grasp, and maintaining stability of clients legs by standing KNEE TO KNEE AND TOE TO TOE
- Walks slightly behind and to the side of client for a distance of 10ft while holding onto the belt
- Ask client is they are okay/feeling dizzy/in pain
- Assist client back to bed and make sure the back of their knee is touching the bed before sitting down
- Remove transfer belt
- Make sure bed is locked, in low position, and call light is in reach
- Wash hands, open curtain, record and report
SKILL 4: Assist with use of bed pan
- Sanitize/wash hands and knock before entering
- Introduce yourself and check patients wristband
- Explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible
- Provide privacy with curtain
- Lower head of the bed and make sure bed is in comfortable position.
- Put on clean gloves and lower side rails
- Put patient to their side
- Place under pad under the patient, then bed pan correctly
- Put patient back into supine position and put up side railes
- Remove gloves correctly and wash hands
- Raise head of the bed and lower bed to safe position
- Tissue and hand wipe is within reach and instruct patient to clean hands with hand wipe when finished
- Instruct patient to signal with call light when finished
- Sanitize hands, and knock on the door
- Lower head of the bed, raise bed to comfortable position and lower side rails
- Put on gloves before removing bed pan
- Turn patient to their side while also holding onto the bedpan to prevent spillage
- Remove bedpan and put it to the side in designated dirty area.
- Put patient back into supine position, Remove gloves and sanitize. Put side rails up, lower bed to safe position
- Put on clean gloves and clean and rinse bedpan, pour contents into toilet.
- Place bedpan in dirty supply area
- Removes gloves and dispose. Wash hands
- Call light is in reach, bed is in low position, bed is locked, open curtains, wash hands
record and report
SKILL 5: Cleans upper or lower denture
- Sanitize/wash hands and knock before entering
- Introduce yourself and check patients wristband
- Explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible
- Puts on clean gloves
- Put towel at bottom of the sink and partially fill sink with water before denture is held over the sink
- Rinse denture in moderate temperature running water before brushing the dentures. Place in basin.
- Apply toothpaste on toothbrush
- Brush all surfaces of dentures
- Rinse all surface of dentures under moderate temperature running water. Place in basin
- Rinses denture cup and lid
- Place denture in denture cup with moderate temperature water/solution and place lid on cup
- Rinse toothbrush and place it in designated toothbrush basin/container
- Maintains clean technique with placement of toothbrush and denture
- Sink liner is removed and disposed of appropriately and/or sink is drained
- Removes gloves and wash hands
SKILL 6: Counts and records radial pulse
- Sanitize/wash hands and knock before entering
- Introduce yourself and check patients wristband
- Explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible
- Close curtain for privacy
6.Place fingertips on thumb side of clients wrist to locate radial pulse
- Counts beats for one full minute
- Make sure call light is in reach
- Wash hands, close curtain, record and report
AVERAGE BPM 60-100
SKILL 7: Counts and records respiration
- Sanitize/wash hands and knock before entering
- Introduce yourself and check patients wristband
- Explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible
- Close curtain for privacy
- Counts respiration for one full minute
- Make sure call light is in reach
- Wash hands, open curtain, record and report
AVERAGE BREATHES PER MINUTE 12-20
SKILL 8: Donning and removing PPE (Gown and gloves)
- Pick up gown and unfold
- Tie neck ties first, then put arms through the sleeves
- Make sure the back is overlapping one another and tie back
- Put on gloves, making sure gloves are over the sleeves of gown
5.BEFORE REMOVING GOWN, WITH ONE HAND, GRAPS THE OTHER GLOVE AT THE PALM AND REMOVE GLOVE
- SLIPS FINGERS FROM UNGLOVED HAND UNDERNEATH CUFF OF REMAINING GLOVE AT WRIST, AND REMOVES GLOVES TURNING IT INSIDE OUT AS IT IS REMOVED
- Disposes gloves into garbage without contaminating self
- Unfasten ties at the back of gown and neck
- Removes gown at the inside of the collar at the neck, pulling away without touching the outside of the gown.
- Roll gown away from body, inward and keeping it inside out without touching ground.
11.Disposes gown into designated garbage
- After completing skill wash hands
SKILL 9: Dresses client with affected (Weak right arm)
SKILL 10: Feeds client who cannot feed self
SKILL 11: Gives modified bed bath (face and one arm, hand and underarm)
SKILL 13: Measures and records urinary output
SKILL 14: Measures and records weight of ambulatory client
SKILL 15: Performs modified passive range of motion for one knee and one ankle
SKILL 16: Performs modified passive range of motion for one shoulder
