Supplies - barrier for table, bath basin, privacy blanket, gloves, chucks, 4 washcloths, soap, and a towel

  1. hand Sanitize
  2. fill basin w/ warm water, ask resident is temp is okay
  3. RAISE BED HEIGHT
  4. put privacy blanket on resident and pull top sheet down to feet
  5. put on gloves
  6. place chucks under resident
  7. wet FIRST WASHCLOTH, make three leaves, and apply soap
  8. separate labia
  9. with one leaf, clean one side of labia front to back
  10. with second leaf. clean other side of labia front to back
  11. with third leaf, clean vaginal area front to back
  12. wet SECOND WASHCLOTH, make three leaves
  13. with one leaf, rinse one side of labia front to back
  14. with second leaf, rinse other side of labia front to back
  15. with third leaf, rinse vaginal area front to back
  16. pat dry with towel
  17. assist resident to turn on side, facing away
  18. wet THIRD WASHCLOTH, make two leaves, and apply soap
  19. with one leaf, wash from vaginal to rectal area
  20. with second leaf, wash from vaginal to rectal area
  21. with FOURTH WASHCLOTH, wet and make two leaves
  22. with one leaf, rinse from vaginal to rectal area
  23. with second leaf, rinse from vaginal to rectal area
  24. pat dry with clean portion of towel
  25. safely remove chucks from under resident
  26. position resident on back, in center of bed
  27. place washcloths and towel in dirty linen
  28. bring basin to sink; rinse and dry basin, using paper towel to touch basin
  29. pick up soap with paper towel and place in basin, bring both back to storage
  30. throw away paper towels and barrier
  31. remove gloves
  32. remove pull up top sheet while removing privacy blanket, and place privacy blanket in dirty linen
  33. LOWER BED HEIGHT
  34. place call light within reach of resident
  35. wash hands

  7. roll privacy blanket up to resident’s waist, exposing only catheter area
  8. wet FIRST WASHCLOTH, make 2 leaves, and apply soap
  9. while holding catheter, use first leaf to wipe 3 - 4 inches down drainage tube
  10. while holding catheter, use second leaf to wipe 3 - 4 inches down drainage tube
  11. wet SECOND WASHCLOTH and make 2 leaves
  12. while holding catheter, use first leaf to rinse 3 - 4 inches down drainage tube
  13. while holding catheter, use second leaf to rinse 3 - 4 inches down drainage tube
  14. using THIRD WASHCLOTH, pat dry 3 - 4 inches down drainage tube
  15. unroll privacy to fully cover resident
  16. safely remove chucks from under resident
  17. place washcloths in dirty linen
  18. bring basin to sink; rinse and dry basin, using paper towel to touch basin
  19. pick up soap with paper towel and place in basin, bring both back to storage
  20. throw away paper towels and barrier
  21. remove gloves
  22. pull up top sheet while removing privacy blanket, and place privacy blanket in dirty linen
  23. LOWER BED HEIGHT

  7. wet FIRST WASHCLOTH and make 4 leaves (NO SOAP)
  8. using first leaf, wash one eye from inner to outer aspect
  9. using second leaf, wash other eye from inner to outer aspect
  10. using third leaf, wash one side of the face
  11. using fourth leaf, wash the other side of the face
  12. pat dry face with FIRST TOWEL
  13. place SECOND TOWEL under resident’s arm, and pull privacy blanket to only expose resident’s arm
  14. wet SECOND WASHCLOTH and apply soap
  15. wash from arm, to hand, and then to underarm
  16. wet THIRD WASHCLOTH
  17. rinse from arm, to hand, and then to underarm
  18. using FIRST TOWEL, pat dry arm, hand, and underarm
  19. remove SECOND TOWEL from under resident’s arm
  20. take off FIRST PAIR OF GLOVES
  21. put on SECOND PAIR OF GLOVES
  22. put clean gown on resident
  23. place washcloths in dirty linen
  24. bring basin to sink; rinse and dry basin, using paper towel to touch basin
  25. pick up soap with paper towel and place in basin, bring both back to storage
  26. throw away paper towels and barrier
  27. remove gloves
  28. pull up top sheet while removing privacy blanket, and place privacy blanket in dirty linen
  29. LOWER BED HEIGHT

  3. apply denture paste to denture brush and put in emesis basin
  4. bring emesis basin, gloves, and washcloth to sink
  5. line the bottom of sink with washcloth
  6. put on gloves
  7. remove denture from cup
  8. using paper towel, turn on the cold water knob of sink
  9. rinse denture under cool water
  10. brush inner surfaces of upper or lower denture
  11. brush outer surfaces of upper or lower denture
  12. brush chewing surfaces of upper or lower denture
  13. rinse all surfaces of denture under cool water
  14. rinse denture cup and lip
  15. place denture into rinsed denture cup
  16. add cool clean water to denture cup and replace lip on denture cup
  17. place denture cup in safe spot
  18. rinse, dry, and return equipment to storage
  19. place sink liner (washcloth) in dirty linen
  20. throw barrier in trash
  21. remove gloves
  22. hand sanitize

  5. gently brush the inner surfaces of resident’s upper and lower teeth
  6. gently brush the outer surfaces of resident’s upper and lower teeth
  7. gently brush chewing surfaces of resident’s upper and lower teeth
  8. gently brush resident’s tongue
  9. have resident spit toothpaste into emesis basin
  10. give resident cup of water to rinse, and have them spit in emesis basin
  11. wipe resident’s mouth with towel
  12. remove towel and place in dirty linen
  13. bring emesis basin and toothbrush to the sink
  14. rinse, dry and return emesis basin and toothbrush to storage; picking up toothpaste along the way

  3. put on gloves: cuffs of gloves overlap cuffs of gown
  4. place barrier on floor under drainage bag
  5. place graduate on barrier
  6. open drain, allowing urine to flow into graduate until bag is empty
  7. close drain
  8. wipe drain w/ alcohol wipe before placing drain back into bag
  9. place graduate on flat surface
  10. measure output
  11. empty graduate into toilet
  12. rinse graduate, emptying rinse water into toilet
  13. return graduate to storage
  14. remove and dispose of gloves
  15. remove gown: remove gown at the neck, then waist, and finally remove whole gown by rolling soiled area to soiled area, and dispose of gown
  16. record output in mL’s
  17. place call light within reach of resident

  8. wet FIRST WASHCLOTH, and apply soap
  9. using first washcloth, wash entire foot, getting between toes as well
  10. place resident’s foot back into bath basin
  11. wet SECOND WASHCLOTH
  12. using second washcloth, rinse resident’s entire foot, getting between toes as well
  13. place resident’s foot back onto towel
  14. dry resident’s entire foot, getting between toes as well
  15. apply lotion to your hands and warm it up by rubbing hands together
  16. apply lotion to top and bottom of resident’s foot, avoid getting lotion between toes
  17. using the towel, wipe off any excess lotion from resident’s foot
  18. replace resident’s sock

  7. give resident paper towel to dry hands
  8. sit in chair facing the resident
  9. describe the food and fluid being offered
  10. offer small amounts of food at a reasonable rate, with time for the resident to chew and swallow; offer fluid after each bite
  11. wipe resident’s hands and mouth after feeding
  12. dispose of wipes in trash
  13. remove clothing protector and place in dirty linen
  14. leave resident sitting upright; but lower head of bed to at least 45 degrees
  15. record intake for solid food and fluid consumed, in mL’s

  8. slide the rest of the stocking over the foot
  9. smooth the stocking the rest of the way up the leg
  10. make sure stocking is wrinkle-free
  11. cover resident’s leg w/ top sheet
  12. place call light within reach of resident

  7. button all of the buttons on resident’s shirt and smooth it out
  8. pull privacy blanket up to resident’s ankles
  9. put socks on resident
  10. smooth out resident’s socks
  11. scrunch up resident’s pants and place around resident’s ankles
  12. have resident raise their hips while you pull their pants up and over their hips
  13. remove privacy blank and place in dirty linen
  14. place call light within reach of resident

  5. roll soiled bottom sheet to the center of bed
  6. tuck soiled bottom sheet under resident
  7. place clean bottom sheet on bed and secure two fitted corners
  8. roll clean bottom sheet and tuck under soiled sheet
  9. roll resident on their other side
  10. remove soiled bottom sheet and place in dirty linen
  11. roll out the rest of clean bottom sheet and smooth out to ensure no wrinkles
  12. secure other two fitted corners
  13. place resident on their back
  14. place clean top sheet over resident
  15. remove privacy blanket and place in dirty linen
  16. place clean bedspread/blanket over clean top sheet
  17. tuck in clean top sheet at the foot of the bed, while providing room for resident’s feet to move freely
  18. remove pillow from under resident
  19. take off soiled pillow case and place in dirty linen
  20. apply clean pillow case w/ tags/zippers on inside
  21. gently lift resident’s head and replace pillow
  22. place call light within reach of resident

  1. hand sanitize
  2. place privacy blanket over resident
  3. pull top sheet down to foot of the bed
  4. direct RN Observer to stand on opposite of working side or raise bed rail on opposite side
  5. have resident scoot toward side of bed closest to you
  6. assist resident to move furthest arm up and behind head (making a triangle)
  7. assist resident to bend closest knee w/ foot flat on bed, and angle furthest knee out (making a triangle)
  8. turn resident on their side
  9. take first pillow and put at angle against the resident’s back, then tuck top edge of pillow underneath resident by pushing down on the pillow and under the resident’s back
  10. tuck the bottom edge of the pillow under resident’s back as well by rolling up bottom edge and pushing under resident’s back
  11. take the second pillow and position it between resident’s legs, by lifting top leg and laying pillow lengthwise between resident’s two legs; specifically between the knees and ankles
  12. take the third pillow and place it underneath the upper arm
  13. adjust pillow underneath resident’s head to make sure it’s not under the shoulder and remains only under head and neck; move the arm next to head under the pillow for a more comfortable position
  14. remove privacy blanket, being careful not to dislodge any of the pillow; place in dirty linen
  15. pull up top sheet to cover resident
  16. place call light within reach of resident

  3. assist resident to put on non-skid footwear
  4. adjust bed height to ensure resident’s feet are flat on the floor when resident is sitting
  5. place wheelchair facing the resident, and at the foot of the bed
  6. lock wheelchair breaks to ensure resident’s safety; remove leg rests for resident’s safety
  7. lock bed brakes to ensure resident’s safety
  8. bring resident to a seated position; while lying down in bed, have resident cross their arms together, put on arm under resident’s upper back/ shoulder area and the other arm under resident’s opposing knee, bring resident to a seated position in one motion
  9. ASK RESIDENT IF THEY ARE DIZZY
  10. place gait belt around resident’s waist
  11. ensure gait belt is tight but not too tight by putting 4 fingers under gait belt
  12. face the resident and make sure both feet are flat on the ground and touching
  13. position your left foot sideways, in-front of resident’s feet; and your right foot facing forwards of the right side of patient’s legs
  14. grasp gait belt on both sides w/ an upward grasp
  15. give the resident a countdown and bring them to their feet on “1”
  16. ambulate resident 5 feet away from wheelchair and 5 feet back towards wheelchair
  17. move resident backwards until the back of their legs touch the front of the wheelchair
  18. give resident a countdown and sit them down in wheelchair in a controlled manner on “1”
  19. remove gait belt from resident
  20. unlock wheelchair breaks and face resident towards the front of room
  21. lock wheelchair breaks again
  22. place leg rests back onto wheelchair and place resident’s feet on leg rests
  23. place call light within reach of resident
  24. discard of barrier in trash
  25. hand sanitize

  4. adjust bed height to ensure resident’s feet are flat on the floor when resident is sitting
  5. place wheelchair facing the resident, and at the foot of the bed
  6. lock wheelchair breaks to ensure resident’s safety; remove leg rests for resident’s safety
  7. lock bed brakes to ensure resident’s safety
  8. bring resident to a seated position; while lying down in bed, have resident cross their arms together, put on arm under resident’s upper back/ shoulder area and the other arm under resident’s opposing knee, bring resident to a seated position in one motion
  9. ASK RESIDENT IF THEY ARE DIZZY
  10. place gait belt around resident’s waist
  11. ensure gait belt is tight but not too tight by putting 4 fingers under gait belt
  12. face the resident and make sure both feet are flat on the ground and touching
  13. position your left foot sideways, in-front of resident’s feet; and your right foot facing forwards of the right side of patient’s legs
  14. grasp gait belt on both sides w/ an upward grasp
  15. give the resident a countdown and bring them to their feet, pivot them, and sit them in wheelchair in a controlled manner on “1”
  16. remove gait belt from resident
  17. unlock wheelchair breaks and face resident towards the front of room
  18. lock wheelchair breaks again
  19. place leg rests back onto wheelchair and place resident’s feet on leg rests
  20. place call light within reach of resident
  21. discard of barrier in trash
  22. hand sanitize

  5. gently complete abduction and adduction of the hip 3 time ASK ABOUT PAIN LEVELS
  6. gently bend the resident’s knee and hip towards the resident’s trunk (flexion) ASK ABOUT PAIN LEVELS
  7. gently straighten the resident’s knee and hip (extension) ASK ABOUT PAIN LEVELS

  4. gently return resident’s arm back down to side of resident’s body (extension) *ASK ABOUT PAIN LEVELS
  5. gently complete flexion and extension of the shoulder 3 times *ASK ABOUT PAIN LEVELS
  6. gently move the resident’s entire arm away from the resident’s body (abduction) *ASK ABOUT PAIN LEVELS
  7. gently return resident’s arm to the side of their body (adduction) *ASK ABOUT PAIN LEVELS
