- fill basin w/ warm water, ask resident is temp is okay
- RAISE BED HEIGHT
- put privacy blanket on resident and pull top sheet down to feet
- put on gloves
- place chucks under resident
- wet FIRST WASHCLOTH, make three leaves, and apply soap
- separate labia
- with one leaf, clean one side of labia front to back
- with second leaf. clean other side of labia front to back
- with third leaf, clean vaginal area front to back
- wet SECOND WASHCLOTH, make three leaves
- with one leaf, rinse one side of labia front to back
- with second leaf, rinse other side of labia front to back
- with third leaf, rinse vaginal area front to back
- pat dry with towel
- assist resident to turn on side, facing away
- wet THIRD WASHCLOTH, make two leaves, and apply soap
- with one leaf, wash from vaginal to rectal area
- with second leaf, wash from vaginal to rectal area
- with FOURTH WASHCLOTH, wet and make two leaves
- with one leaf, rinse from vaginal to rectal area
- with second leaf, rinse from vaginal to rectal area
- pat dry with clean portion of towel
- safely remove chucks from under resident

- put privacy blanket on resident and pull top sheet down to feet
- put on gloves
- place chucks under resident
- roll privacy blanket up to resident’s waist, exposing only catheter area
- wet FIRST WASHCLOTH, make 2 leaves, and apply soap
- while holding catheter, use first leaf to wipe 3 - 4 inches down drainage tube
- while holding catheter, use second leaf to wipe 3 - 4 inches down drainage tube
- wet SECOND WASHCLOTH and make 2 leaves
- while holding catheter, use first leaf to rinse 3 - 4 inches down drainage tube
- while holding catheter, use second leaf to rinse 3 - 4 inches down drainage tube
- using THIRD WASHCLOTH, pat dry 3 - 4 inches down drainage tube
- unroll privacy to fully cover resident
- safely remove chucks from under resident
- place washcloths in dirty linen
- bring basin to sink; rinse and dry basin, using paper towel to touch basin
- pick up soap with paper towel and place in basin, bring both back to storage
- throw away paper towels and barrier
- remove gloves
- pull up top sheet while removing privacy blanket, and place privacy blanket in dirty linen

- using second leaf, wash other eye from inner to outer aspect
- using third leaf, wash one side of the face
- using fourth leaf, wash the other side of the face
- pat dry face with FIRST TOWEL
- place SECOND TOWEL under resident’s arm, and pull privacy blanket to only expose resident’s arm
- wet SECOND WASHCLOTH and apply soap
- wash from arm, to hand, and then to underarm
- wet THIRD WASHCLOTH
- rinse from arm, to hand, and then to underarm
- using FIRST TOWEL, pat dry arm, hand, and underarm
- remove SECOND TOWEL from under resident’s arm
- take off FIRST PAIR OF GLOVES
- put on SECOND PAIR OF GLOVES
- put clean gown on resident
- place washcloths in dirty linen
- bring basin to sink; rinse and dry basin, using paper towel to touch basin
- pick up soap with paper towel and place in basin, bring both back to storage
- throw away paper towels and barrier
- remove gloves
- pull up top sheet while removing privacy blanket, and place privacy blanket in dirty linen

- hand sanitizer
- bring denture cup to the sink
- apply denture paste to denture brush and put in emesis basin
- bring emesis basin, gloves, and washcloth to sink
- line the bottom of sink with washcloth
- put on gloves
- remove denture from cup
- using paper towel, turn on the cold water knob of sink
- rinse denture under cool water
- brush inner surfaces of upper or lower denture
- brush outer surfaces of upper or lower denture
- brush chewing surfaces of upper or lower denture
- rinse all surfaces of denture under cool water
- rinse denture cup and lip
- place denture into rinsed denture cup
- add cool clean water to denture cup and replace lip on denture cup
- place denture cup in safe spot
- rinse, dry, and return equipment to storage
- place sink liner (washcloth) in dirty linen
- throw barrier in trash

Mouth Care - Brushing Resident’s Teeth

- put towel around resident’s chest
- put on gloves
- wet toothbrush and apply small amount of toothpaste
- gently brush the inner surfaces of resident’s upper and lower teeth
- gently brush the outer surfaces of resident’s upper and lower teeth
- gently brush chewing surfaces of resident’s upper and lower teeth
- gently brush resident’s tongue
- have resident spit toothpaste into emesis basin
- give resident cup of water to rinse, and have them spit in emesis basin
- wipe resident’s mouth with towel
- remove towel and place in dirty linen
- bring emesis basin and toothbrush to the sink
- rinse, dry and return emesis basin and toothbrush to storage; picking up toothpaste along the way
- throw barrier in trash
- remove gloves

- return equipment to storage
- take off SECOND PAIR OF GLOVES
- fill bath basin with water
- wet FIRST WASHCLOTH, apply soap

- put on gown: unfold gown, face back of opening, place arms through each sleeve, secure neck opening, secure gown at waist
- put on gloves: cuffs of gloves overlap cuffs of gown
- place barrier on floor under drainage bag
- place graduate on barrier
- open drain, allowing urine to flow into graduate until bag is empty
- close drain
- wipe drain w/ alcohol wipe before placing drain back into bag
- place graduate on flat surface
- measure output
- empty graduate into toilet
- rinse graduate, emptying rinse water into toilet
- return graduate to storage
- remove and dispose of gloves
- remove gown: remove gown at the neck, then waist, and finally remove whole gown by rolling soiled area to soiled area, and dispose of gown
- record output in mL’s
- place call light within reach of resident
- wash hands
Foot Care on One Foot

- immerse resident’s foot in warm water
- verbalize the 5 TO 20 MINUTE SOAKING TIME
- place resident’s foot on towel
- wet FIRST WASHCLOTH, and apply soap
- using first washcloth, wash entire foot, getting between toes as well
- place resident’s foot back into bath basin
- wet SECOND WASHCLOTH
- using second washcloth, rinse resident’s entire foot, getting between toes as well
- place resident’s foot back onto towel
- dry resident’s entire foot, getting between toes as well
- apply lotion to your hands and warm it up by rubbing hands together
- apply lotion to top and bottom of resident’s foot, avoid getting lotion between toes
- using the towel, wipe off any excess lotion from resident’s foot
- replace resident’s sock
- empty, rinse, and return bath basin to storage; picking up soap and lotion along the way
- place washcloths and towel in dirty linen
- throw barrier in trash

Nail Care on One Hand

- position resident in an upright sitting position before feeding (75 - 90 degrees)
- place towel around resident’s chest to protect clothing before feeding
- give resident disposable wipe to clean hands
- after use, dispose of wipe in trash
- give resident paper towel to dry hands
- sit in chair facing the resident
- describe the food and fluid being offered
- offer small amounts of food at a reasonable rate, with time for the resident to chew and swallow; offer fluid after each bite
- wipe resident’s hands and mouth after feeding
- dispose of wipes in trash
- remove clothing protector and place in dirty linen
- leave resident sitting upright; but lower head of bed to at least 45 degrees
- record intake for solid food and fluid consumed, in mL’s

- put hand in stocking until heel square is in the palm of hand
- hold heel square by making a fist
- turn stocking inside out, while still holding heel square at the back of stocking
- scrunch sock up to the toes, keeping heel square at the back of sock
- put the sock over the toes, heel square first, until the heel square is over heel
- slide the rest of the stocking over the foot
- smooth the stocking the rest of the way up the leg
- make sure stocking is wrinkle-free
- cover resident’s leg w/ top sheet
- place call light within reach of resident

Dressing a Resident w/ an Affected (Weak) Side - shirt, pants, and socks

- dress the resident in button-up shirt, dressing the affected side first; insert hand through sleeve of shirt and grasp resident’s hand while putting shirt on
- button all of the buttons on resident’s shirt and smooth it out
- pull privacy blanket up to resident’s ankles
- put socks on resident
- smooth out resident’s socks
- scrunch up resident’s pants and place around resident’s ankles
- have resident raise their hips while you pull their pants up and over their hips
- remove privacy blank and place in dirty linen
- place call light within reach of resident

- roll soiled bottom sheet to the center of bed
- tuck soiled bottom sheet under resident
- place clean bottom sheet on bed and secure two fitted corners
- roll clean bottom sheet and tuck under soiled sheet
- roll resident on their other side
- remove soiled bottom sheet and place in dirty linen
- roll out the rest of clean bottom sheet and smooth out to ensure no wrinkles
- secure other two fitted corners
- place resident on their back
- place clean top sheet over resident
- remove privacy blanket and place in dirty linen
- place clean bedspread/blanket over clean top sheet
- tuck in clean top sheet at the foot of the bed, while providing room for resident’s feet to move freely
- remove pillow from under resident
- take off soiled pillow case and place in dirty linen
- apply clean pillow case w/ tags/zippers on inside
- gently lift resident’s head and replace pillow
- place call light within reach of resident

Positioning a Dependent Resident in Bed on Side

- hand sanitize
- place privacy blanket over resident
- pull top sheet down to foot of the bed
- direct RN Observer to stand on opposite of working side or raise bed rail on opposite side
- have resident scoot toward side of bed closest to you
- assist resident to move furthest arm up and behind head (making a triangle)
- assist resident to bend closest knee w/ foot flat on bed, and angle furthest knee out (making a triangle)
- turn resident on their side
- take first pillow and put at angle against the resident’s back, then tuck top edge of pillow underneath resident by pushing down on the pillow and under the resident’s back
- tuck the bottom edge of the pillow under resident’s back as well by rolling up bottom edge and pushing under resident’s back
- take the second pillow and position it between resident’s legs, by lifting top leg and laying pillow lengthwise between resident’s two legs; specifically between the knees and ankles
- take the third pillow and place it underneath the upper arm
- adjust pillow underneath resident’s head to make sure it’s not under the shoulder and remains only under head and neck; move the arm next to head under the pillow for a more comfortable position
- remove privacy blanket, being careful not to dislodge any of the pillow; place in dirty linen
- pull up top sheet to cover resident
- place call light within reach of resident

Assist Resident to Ambulate using Gait Belt

- assist resident to put on non-skid footwear
- adjust bed height to ensure resident’s feet are flat on the floor when resident is sitting
- place wheelchair facing the resident, and at the foot of the bed
- lock wheelchair breaks to ensure resident’s safety; remove leg rests for resident’s safety
- lock bed brakes to ensure resident’s safety
- bring resident to a seated position; while lying down in bed, have resident cross their arms together, put on arm under resident’s upper back/ shoulder area and the other arm under resident’s opposing knee, bring resident to a seated position in one motion
- ASK RESIDENT IF THEY ARE DIZZY
- place gait belt around resident’s waist
- ensure gait belt is tight but not too tight by putting 4 fingers under gait belt
- face the resident and make sure both feet are flat on the ground and touching
- position your left foot sideways, in-front of resident’s feet; and your right foot facing forwards of the right side of patient’s legs
- grasp gait belt on both sides w/ an upward grasp
- give the resident a countdown and bring them to their feet on “1”
- ambulate resident 5 feet away from wheelchair and 5 feet back towards wheelchair
- move resident backwards until the back of their legs touch the front of the wheelchair
- give resident a countdown and sit them down in wheelchair in a controlled manner on “1”
- remove gait belt from resident
- unlock wheelchair breaks and face resident towards the front of room
- lock wheelchair breaks again
- place leg rests back onto wheelchair and place resident’s feet on leg rests
- place call light within reach of resident
- discard of barrier in trash

Transfer Resident from Bed to Wheelchair using a Gait Belt

- locate resident’s radial pulse by placing fingertips on thumb side of resident’s wrist
- count resident’s radial pulse for one full minute; tell RN Observer when you start and stop counting
- record resident’s radial pulse rate reading on documentation sheet
- count resident’s respirations for one full minute; tell RN Observer when you start and stop counting
- record resident’s respiration reading on documentation sheet
- place call light within reach of resident
