What is the FDA approved indication for Januvia?
As an adjunct to diet and exercise to improve glycemic control and patient with type two diabetes mellitus as monotherapy or as combination therapy with metformin or peroxisome proliferator-activated receptor gamma agonist when the single agent does not provide adequate glycemic control
What is the dosage of Januvia?
The normal adult dose is 100 mg once daily.
In patients with renal impairment, (CrCl 30-50 mL/min) 50 mg once daily
In patients with renal impairment, (CrCl >30mL/min) 25 mg once daily
What should the providers monitor when their patient is on sitagliptin Phosphate?
Fasting Blood Glucose, HbA-1c
What is the mechanism of action for Januvia?
Sitagliptin inhibits the dipeptidyl peptidase-4 (DDP-4) enzyme.
What are the drug interactions for Januvia?
May increase the peak drug concentration of digoxin.
What are the contraindications of Januvia?
- Contraindicated in patients have shown hypersensitivity to any ingredients.
- Use with caution in patients with renal insufficiency.
- The use of sitagliptin phosphate with drugs which may cause hypoglycemic reactions, including sulfonylurea or insulin, has not been adequately studied & this combination should be avoided.
- Pregnancy Category B
What are the adverse effects of Januvia?
Nasopharyngitis
What should the pharmacist tell the patient when counseling them on the use of Januvia?
- May be taken without regard to meals.
- Closely follow recommended dietary and exercise instructions.
- Quarterly HbA1-c testing should be performed.
- If a dose is missed, take it as soon as possible but do not double doses.
