give some examples of differential diagnoses for shoulder pain, other than MSK
cardiac, gallbladder, coming from neck, polymyalgia
what is involved in the look section of the examination of the shoulder
watch how patient undresses, check both sides(symmetry), deformity, scars
what is involved in the feel section of the examination
bony landmarks, any tenderness, check axilla, get patient to pinpoint site of pain
what is involved in the move section of the examination
abduction(check scapular movement), active + passive, external rotation, internal rotation
what investigations may be used for shoulder pain
FBC(infection?), viscosity, X-ray, USS(soft tissue?)
what is involved in the non-pharmacological treatment of shoulder pain
explanation, mobilise, physio, time
what is involved in the pharmacological treatment of shoulder pain
NSAIDs(short term), local injection
what are the common shoulder problems seen in GP
rotator cuff problems(esp. supraspinatus tendonitis), sub-acromial bursitis, acromioclavicular disease
what are some less common shoulder problems seen in GP
frozen shoulder, OA or RA of the shoulder, recurrent dislocation
