What is the pathogenesis of a UTI?

A

*most urinary pathogens originate in rectal flora and enter bladder via urethra
*lower urethra contaminated with microorganisms
*disturbances to host defences allows ascending infection
*microbes have virulence factors to overcome host defences

What are virulence factors for E. coli?

A

*flagella- motility
*aerobactin- Fe acquisition in Fe poor urinary tract
*haemolysin- form pores in target cells
*presence of adhesions on fimbriae and cell surface
(Two types of E. coli- type 1 fimbriae =cystitis, type P fimbriae= pyelonephritis)

A

*dysuria- burning and stinging on urination
*increased frequency
*nocturia-need to get up in night frequently
*haematuria-blood in urine

How are UTI’s diagnosed?

A

*dipstick test for nitrite in urine- pathogens catalyse reduction of nitrate to nitrite (urine must be in bladder for >4 hours)
*test for leukocyte esterase (LE)- esterase released by Leukos when pyuria (puss in urine) present
NB negative nitrite test doesn’t mean no UTI- need to test LE
*in under 65’s- 2 or more symptoms plus positive dipstick nitrite test

When is pyelonephritis common?

A

Asymptomatic bacteriuria in pregnant women
Failure to treat= premature labour

What is first line treatment of acute uncomplicated lower UTI?

A

Ibuprofen 400mg one three times daily for 3 days
Antibiotic- nitrofurantonin 100mg MR tabs twice daily
Or
Trimethoprim 200mg twice daily for 3 days

What are reserved 2nd line treatments for acute uncomplicated lower UTI?

A

Amoxicillin
Co-amoxiclav
Pivemecillinam

How is acute upper pyelonephritis treated?

A

Non pregnant- 7 day course of ciprofloxacin or 14 day course of co-amoxiclav

How are recurrent UTI’s treated?

A

Relapse- search for cause. Oral antibiotics for 4-6 weeks or long term
Reinfection- low dose prophylaxis of trimethoprim 100mg or nitrofuratonin 50mg for 6-12 months

What are tips for prophylaxis of UTI’s?

A

*2.5L water daily
*avoid constipation
*cranberry juice
*paracetamol/ibuprofen for pain
*urinate at bedtime and after sex
