What are the aims of treatment for SBS?
Ensuring adequate nutrition and drug absorption
What drug treatments may be required for SBS?
Vitamin supplement’s
Oral rehydration salt’s(diarrhea is common)
Anti motility drug (loperamide>codeine phosphate)
Colestyramine (binds the unabsorbed bike salt’s and reduce diarrhea)
Drugs that reduce gastric acid secretion (omeprazole)
Growth factors (teglutide) to facilitate intestinal adaption after surgery
Why would you choose loperamide over codeine for diarrhoea?
Loperamide has less side effects, it is not sedative and does not cause dependence codeine can be added if loperamide doesn’t work.
What would you look out for in a patient taking colestyramine?
Fat malabsorption or fat soluble vitamin deficiencies
Many drugs are incompletely absorbed by patints with a Short bowel. What aspects of formulation is most suitable? (More than one)
Enteric coated Modified release Effervescent/soluble Liquids Controlled release Uncoated
Soluble, liquid and uncoated formulations are most suitable. As the small intestines are affected there may not be sufficient length of bowel to allow full release of the drug. Dosage forms with quick dissolution should be used. Uncoated and liquids are also suitable.
