Grade 1 murmur
very soft & INTERMITTENT (heard then not heard later)
Grade 2 murmur
soft murmur consistently heard in a quiet room
Grade 3 murmur
Moderate intensity murmur - think of 3 first!!! if you can hear a murmur its at least a 3
Grade 4 murmur
Loud murmur that RADIATES TO OTHER SIDE OF BODY
Grade 5 murmur
Loud murmur with Precordial Thrill
Grade 6 murmur
Very loud murmur that you can hear with stethoscope off of the body wall
Basic cardiac workup (4)
- thoracic rads
- blood pressure
- Echocardiogram/ultrasound
- ECG
what diagnostic should be run first?
radiographs - CHF congestion will be in the lungs
Full cardiac workup (12)
- thyroid fx - hyperthyroid can cause heart dz in cats
- HW test
- CBC - anemia, causes less viscous, turbulent flow
- Chem/UA - electrolytes, renal function, glucose
- Blood culture - endocarditis
- Thoracic rads
- echocardiogram
- BP
- ECG
- Neuro exam - syncope vs seizure
- Holter monitoring
- Event monitoring

- eyes - blindness, retinal detachment (esp cats)
- kidneys - glomerular sclerosis, ↓ nephrons
- heart - ↑ afterload to the heart causing left ventricular hypertrophy or cardiac failure
- brain - encephalopathy or stroke
