How to assess a woman requesting contraception?
Her preferred method and future plans of children etc.
Identify comorbidities:
HTN, migrains, allergies, smoking, reproductive hx, drug hx.
Assess risk of STI, sexual abuse
What is the Fraser criteria?
It is lawful to provide contraception advice and tx to young people without parental consent if Fraser criteria met:
Understands practitioners advice.
Likely to begin or continue having sex.
Ppl >16 presumed to be competent to consent. If under 16 then competence must be demonstrated.
What are the different types of intrauterine contraception?
Copper-bearing IUD
Levonorgestrel-releasing IUS:
Available in 2 doses, 13.5mg licensed for 3 years.
52mg licensed for 5 yrs.
Protection and risks of intrauterine contraception?
When can you not use these?
Benefit of contraception outweighs risk:
Hx, fx of VTE, impaired cardiac function, AF,
DO NOT USE:
stroke, long QT syndrome, unexplained vaginal bleeding, cervical cancer, distorted uterine cavity, PID, symptomatic STIs
Progesterone only contraceprtion examples?
POP:
Contain either norethisterone
Desogestrel or levonorgestrel.
