Systemic Inflammatory Response System
What is Sepsis?
SIRS positive & Infection
What is the SIRS criteria?
2 OR MORE OF: Temp >38 or <34 HR >90 RR >20 WBC >12000cells/mm2 or <4000cells/mm2
What is severe sepsis?
Sepsis + end organ damage
What is a classic blood finding in severe sepsis?
Elevated serum lactate
What is septic shock?
Severe sepsis + Hypotension (requiring vasopressors to keep above 65mmHg)
What is the hospital mortality in septic shock?
40%
In any patient with an infection, what assessment should be done? And what similar assessment if it is an emergency?
SOFA
qSOFA
How should sepsis be managed?
Sepsis 6:
Blood cultures
Measure and monitor urine output
Blood lactate - general indicator and marker of poor prognosis
Antibiotics dependant on case history
O2 - stats need to stay above 94%
IV Fluid challenge
What are some indicators of severe sepsis?
low BP that is not responding to fluids lactate <2 increased CCK oliguria liver dysfunction
