Sepsis Flashcards
What are the SIRS criteria?
temp over 100.4 F or less than 96.8 (36-38 C)
HR over 90
RR over 20 OR PaCO2 less than 32 mmHg
WBC>12,000 or <4,000 or >10% immature bands
SIRS + Infection = ___
sepsis
What puts someone in the severe sepsis cateogy?
sepsis with induced-organ dysfunction or tissue hypo-perfusion
What are the end-organ dysfunction definition criteria?
SBP <90 or MAP <70 or SBP decrease >40 mmHg
Cr >2.0 or UOP <0.5 ml/kg/hr for >2 hrs
Bili >2
Plt <100,000
Coagulopathy
lactate >2
What is the definition of septic shock?
sepsis with persistently low BP despite fluid administration (30 mL/kg)
What makes a lactate go up in sepsis?
anaerobic glycolysis during periods of insufficient oxygen delivery
What are the four things you must do for the three-hour bundle the sepsis quality measure?
- lactate measurement
- blood cultures before starting abx
- administratin of broad spec ABx
- administration of 30 ml/kg IV crystalloid for hypotension or lactate over 4
What are the three things you must do for the 6 hour bundle in the sepsis quality measure?
- give vasopressors for hypotension that doesn’t respond to fluids to get MAP over 65
- reassess and document volume status after fluid administration
- repeat lactate for patients with initial lactate over 4
