Hypertension
ACEI/ARB: eg Ramipril 1.25-10mg or candesartan 4-32mg
CCB: eg Amlodipine 2.5-10mg
Diuretic: eg hydrochlorothiazide 12.5-25mg
Beta Blocker: eg Atenolol 25-100mg daily
Dyslipidaemia
Statins: eg Atorvastatin 10-80mg
+/-
Ezetimibe 10mg
Bile acid binding resin: eg Cholestyramine 4-8mg
Fibrates: eg fenofibrate 145mg (best triglycerides)
Nicotinic acid (rare) 250mg bd
T2DM
Metformin 500mg bd
Sulfonylureas: eg Gliclazide 40mg daily
DPP4 inhibitors: eg Sitagliptin 100mg daily
Glucagon-like pept agonist: eg exenatide 5mcg BD
Acarbose 50-100mg TDS
Insulin
Stable angina acute
GTN spray 400mcg every 5 min
GTN tabets 600mcg every 5 minutes
Stable angina prevention
Beta Blocker: eg atenolol 25-100mg
CCB: eg diltiazem CR 180-360mg
Nitrates: eg GTN patch or isosorbide mononitrate SR 30-120mg
STEMI acute
Aspirin 300mg
+ Clopidogrel 300-600mg
Alteplase 15mg Bolus then 50mg over 30 minutes then 35mg over 60 minutes
STEMI management
Antiplatelet eg asp/clopid Beta blocker: eg atenolol ACEI: eg ramipril Statin CCB Anticoagulation if severe: eg warfarin
AF
Rate control:
Beta Blocker- aten/metop
Rate CCB
Digoxin
Rhythim control:
Amiodarone 200-400mg TDS for 1 week the BD for 1 week then 100-200mg daily ongoing
Flecinide 50-100mg BD
Haem stroke
Target BP 160/90
Nimodipine 60mg every 4 hours for 21 days if aneurysnal subarachnoid haemorrhage
Ischaemic stroke
Acute:
alteplase 0.9mg/kg up to 90 mg over 1 hour if within 4.5 hours
Aspirin 300mg
Prevention:
Aspirin/Clop
warfarin 5mg daily for 2 days then adjust
Hyperthyroid
Carbimazole 10-45mg daily in 2-3
Or
Propylthiouracil 200-600mg in 2-3
Beta blockers for symptoms
Propranolol 10mg BD
Hypothyroid
Thyroxine 50-100mcg increasing to 100-200 slowly
Pain (Neuropathic)
Amitriptyline 10-25mg at night max 100mg
Gabapentin 100-300mg daily max 2.4g
Pregabalin 75mg max 300
Duloxetine 30mg max 60mg BD
