selective tissue tensioning Flashcards
Describe the inherent likelihood for damage to a contractile tissue
pain with resistance, stretch, and palpation
-applies to muscle or tendon
Describe the inherent likelihood for damage to inert tissue
pain with stretching passively/actively, pain with palpation due to guarding or if superficial
-applies to ligament, joint capsule, nerve, bursae, etc.
what are you looking for during palpation of a STATIONARY joint?
temperature, swelling, gaps, tenderness, pulse
what are you looking for during palpation of a MOVING joint?
crepitus, clicks, end feel, hypermobility/hypomobility, willingness to move, pain
Passive movement can be inhibited by:
ankylosis (fusion)
fibrosis (scar tissue)
muscle spasm
rigidity
What are the likely PROM, AROM, and palpation expectations for muscle tightness?
AROM/PROM: fairly = and/or painful to each other; not really capsular
Palpation -increased tone/tenderness
resistance:strong w/ pain if guarding
What are the likely PROM, AROM, and palpation expectations for joint pain?
PROM/AROM: equally limited and/or painful; potential capsular pattern
Palpation: pain IF superficial
resistance: strong, pain if guarding
What are the likely PROM, AROM, and palpation expectations for muscle or tendon tear?
AROM: limited
PROM: Full, likely w/ pain
Palpation: painful
resistance: weak-painful for partial, unable for full tear
What are the likely PROM, AROM, and palpation expectations for muscle strain or tendonitis?
AROM: painful
PROM: full but pain w/ involved muscle
palpation; pain, maybe with guarding
resistance: often strong but painful (may limit strength)
What are the likely PROM, AROM, and palpation expectations for ligamentous tears or sprains?
AROM/PROM: equally limited and/or painful in direction of restraint
Palpation: painful IF superficial
resistance: strong, only painful if guarded
What are the likely PROM, AROM, and palpation expectations for ligamentous adhesions?
AROM/ PROM: equally limited and/or painful
Palpation: pain IF superficial
resistance: strong unless guarded
What are the likely PROM, AROM, and palpation expectations for capsular hypomobility?
AROM/ PROM: equally limited and/or painful; potential capsular pattern
Palpation: pain IF superficial
resistance: strong unless guarded
what information should you gather during the examination process?
history, condition at rest, active and passive movement, joint mobes, special tests, palpation
what are you looking for during active movement?
pain, ROM, quality of movement and willingness to move
what are you looking for during passive movement?
pain, ROM, willingness to allow movement, end feel (where is it?whenyou feel? Why is important)
