section 10 Flashcards
true auto antibody
develop against factors, altering function or promoting rapid clearence
Factor VIII:C inhibitor
Hemophilia patents: Complicates condition
Leads to acquired hemophilia
Acquired hemophilia
bleeding not as severe, can be fatal
Post-partum, or with SLE, RA, drug rxn etc.
Factor VIII:C inhibitor Lab diag
PT: normal
APTT: prolonged
50:50 mix: prolonged (VIII takes time to react)
Anti-phospholipid antibodies
1-2% of population
transient, disappear in 12 weeks
ass. with SLE,RA, Sjogrens syndrome
Anti-phospholipid syndrome
Thrombosis
Lupus Inhibitor (LI)
Non-specific for any coag factor
Does NOT react with endogenous platelet phospholipid
DOES react with test systems
LI Manifestations
Not ass. w/ bleeding
Prone to thrombosis
Inhibits prostacyclin (normally inhibits plt adhesion to endothelials)
LI lab findings
APTT impacted the most
50:50 APTT: increased
MUST determine if the inhibitor is heparin
LI Lab CONFIRMATION testing
APTT w/ low phospholipid reagent
Dilute RVVT
KCT
Dilute TT
DRVVT confirmation follow-up
Platelet Neutralization Procedure (PNP)
Silica Clotting Time (SCT)
If LI present, result is prolonged longer than normal plasma
DRVVT
Dilute venom
Sensitive to LI
Clotting time increased more than normal plasma
Kaolin Clotting Time (KCT)
LI inhibits
Kaolin initiates contact factors
Prolonged in the presence of LI
Dilute Thromboplastin Inhibition assay
LI inhibits highly dilute Thromboplastin
