Principles of Moist Wound Healing
Full thickness wound kept in moist environment will re-epithelialize within 12-15 days
Wound exudate not lost via absorption by gauze
Wound exudate remains in contact with wound bed
Wound exudate contains healing “rich” properties
What type of debridement dressing should be used with necrotic/heavily exudating wounds?
Hypertonic Saline
Kerlix AMD
Honey
What type of debridement dressing should be used with dry wounds?
Moistening - gel dressings
What type of debridement dressing should be used with granulation tissue defects?
Calcium Alginate: Curasorb
What type of debridement dressing should be used with epithelialization?
Semi-occlusive foam: COPA/hydrasorb
What are examples of debridement dressings?
Saline
Hypertonic saline
Antimicrobial dressing
Honey
When should hypertonic saline dressings (curasalt) be used?
Infected or heavily exudating wounds only!!
When should hypertonic saline dressings be changed?
At least every 3 days to prevent dilution of the hypertonic saline
When should antimicrobial dressings (kerlix AMD) be changed?
Sx incisions every 5 days
Necrotic wounds every 3 days
Premoisten in dry wounds/cover
What are indications for negative pressure wound therapy?
Highly exudative wounds
Wounds with large dead space
Infected wounds
What are examples of moisturizing dressings?
Hydrogels
When should moisturizing dressings be used?
Dry to moist wounds with cavities
First and second degree burns
Cuts, abrasions
Minor irritations of the skin
When should moisturizing dressings be changed?
Can be left on up to 5 days
What are examples of granulation and wound contraction dressings?
Calcium Alginate
