A pharmacist may initiate, adapt or renew a prescription only if:
- it’s in the best interest of the patient
- the pharmacist has sufficient knowledge and skills with respect to the relevant drug and condition
- the pharmacist must be in possession of the existing prescription or have access to the information contained in the original prescription
A pharmacist may adapt a prescription based upon the individual circumstances of the patient by altering the
dose, dosage form, regimen or route of administration to address the patient’s unique needs and circumstances.
A pharmacist may renew a prescription if
for the purpose of continuity of care.
quantity cannot exceed the lesser of: originally prescribed (including any refills) or a twelve month supply (As of December 11, 2020, previously 6 months)
When initiating, adapting or renewing, the pharmacist must ensure the following information is recorded on the prescription:
Patient name, address
Drug name, strength, quantity, dose, directions for use, route, refills
Pharmacist name, address, phone, OCP#
Date
Reference to original rx (including prescriber name and contact)
Pharmacy technicians are able to
provide technical drug info
perform technical checks
accept verbal rx and authorize transfers (excluding narcotics, controlled drugs, benzodiazepines and targeted substances)
accept delegated controlled tasks
perform a procedure on tissue below dermis (if RPh is physically present)
