Policy Development- uses the results of assessment as a guide

Pop diagnosis and Priorities- uses assessment data to determine diagnosis or issues
Outcomes Identification- identifies expceted outcomes for a plan
Planning- develops plan that prescribes strategies and alternatives to attain the exppected outcomes
Implementation- implements identified plan including coordinating care, using strategies for health promotion and teaching, provides consultation for influencing the plan, and appplies PH laws, regulations and policies
Evaluation- of progress toward outcomes
