Schizophrenia
A. Two or more* of the following symptoms, present for a significant portion of time, during a 1-month period
- Only one symptom is necessary if delusions are bizarre OR hallucinations consist of a voice keeping a running commentary or two voices conversing
1. Delusions - Bizarre vs. Non-bizarre
- Referential
- Persecutory
- Grandiose
- Religious
2. Hallucinations
3. Disorganized speech (e.g., frequent derailment or incoherence)
4. Grossly disorganized or catatonic behavior
5. Negative symptoms - Affective flattening
- Alogia (poverty of speech/content)
- Avolition (inability to initiate/pursue activities)
B. Social/occupational dysfunction
C. Continuous signs of disturbance persist for at least 6 months
D. Schizoaffective and Mood Disorder exclusion:
No mood episodes have occurred concurrently with active-phase Sxs OR
If mood episodes occurred concurrently, their total duration has been relatively brief
E. Substance/GMC exclusion
F. Relation to Pervasive Developmental Disorder
If history of PDD, additional dx is only made if prominent delusions or hallucinations are also present for at least a month
Schizo subtypes
Paranoid Type Disorganized Type Catatonic Type Undifferentiated Type Residual Type
Schizoaffective
A. Uninterrupted period of illness, and at some point there is a mood episode concurrent with symptoms meeting Criterion A for Schizophrenia
B. During the same period of illness, there have been delusions or hallucinations for at least 2 weeks in the absence of prominent mood symptoms.
C. Symptoms that meet criteria for a mood episode are present for a substantial portion of the total duration of the active and residual periods of the illness.
D. The disturbance is not due to the direct physiological effects of a substance (e.g., a drug of abuse, a medication) or a general medical condition.

Shared Psychotic Disorder
more than 1 person, shared between 2 or more ppl
Substance-Induced Psychotic Disorder
i.e. withdrawal or intoxication
paranoid subtype
preoccupation w/ delusions or hallucinations, but no disorganized speech, disorganized or catanoic behavior, or flat or inappropriate affect. Has best prognosis
catatonic subtype
dominated by postural and/or movement abnormalities, mutism, or echolalia
disorganized subtype
Has worst prognosis. Disorganized speech, disorganized behavior, and flat or inappopriate affect
undifferentiated subtype
patient doesn’t meet criteria for the previous subtypes but still meets general criteria for schizo
residual subtype
met criteria for schizo in past but no longer do. have prominence of neative symptoms but no prominent delusions, hallucinations catatonic symptoms, or disorganized behavior or speech
negative symptoms
involve a decrease in behavior- i.e. flat affect, anhedonia, and lack of motivation
positive symptoms
involve an excess of ideas, sensory experiences, or behavior, i.e. hallucinations, delusions, and bizarre behaviors
