What are the features of sarcoidosis?
erythema nodosum, bilateral lymphadenopathy, swinging fever, SOB (pulmonary fibrosis), malaise, weight loss
What are the investigations for sarcoidosis?
raised serum ACE hypercalcaemia raised ESR and CRP spirometry (restrictive defect) tissue biopsy (non caseating granulomas)
What is primary tuberculosis?
someone who is exposed to TB for the first time
the initial lesion (ghon complex) usually heals by fibrosis. those who are immunocompromised may develop disseminated miliary TB
What is secondary tuberculosis?
a person who is immunocompromised who develops re-activation of the infection
generally occurs in the apex of the lungs
Where are the extra-pulmonary infection sites of TB?
CNS (tuberculous meningitis) Pott's disease (vertebral bodies) cervical lymph nodes renal GI tract
What investigations can you use to diagnose TB?
Mantoux test
ziehl-nelson staining
interferon-gamma blood test
What is the treatment of active TB?
'RIPE' for first 2 months rifampicin isoniazid pyrazinamide ethambutol
