What glands are affected?
Parotid, Submandibular and Sublingual
What causes it?
It can be infection, inflammation or obstruction and tumours
What affects the Parotid? (5)
Viral Parotitis (mumps), Sialithiasis, Benign and Malignant tumours, Sjogren’s Syndrome, HIV
What affects the Submandibular Gland? (3)
Sialithiasis, Tumours, Sjogren’s
What affects the minor glands? (2)
Mucoceles, tumours
What do Salivary stones contain? (4)
Mucus, cellular debris, calcium and magnesium phoshate
What is sialedonosis?
Generalised hypertrophy of the glands which is often caused by Sjogren’s, Bullaemia, Anorexia, Diabetes and Cushings
Symptoms of Infection (7)
Painful swelling, dry mouth, abnormal taste, discharge, pain when eating, decreased mouth opening, fever
Symptoms of Obstruction (3)
Colicky pain, post prandial swelling, relapsing and remitting symptoms
Symptoms of Tumours (4)
Painless gland swelling, fixation to the skin, ulceration, paraesthesia
What bloods would you do? (6)
FBC, CRP, U&E, Blood cultures, Serology for Virus, Salivary Antibodies
Other investigations (7)
Ultrasound, Fine needle aspiration, biopsy, culture of discharge, sialography, CT, MRI
What is the treatment for infection? (6)
Mumps is self limiting, antibiotics if bacterial, drainage of abscess, warm compresses, paracetamol for fever, analgesics
What is the treatment for obstruction? (5)
May pass spontaneously, hydration, analgesics, endoscopic removal, lithotripsy
What is the treatment for Sialedonosis? (2)
Refer to rheumatology, Pilocarpine to reduce salivation
