Salivary Gland Pathology - SY Flashcards
What fluid makes up the parotid gland?
100% serous
What fluid makes up the submandibular gland?
Mostly serous, some mucous
What fluid makes up the sublingual gland?
Mostly mucous, some serous
What is a mucocele caused by?
Break in ductal system due to trauma
What is present with mucin + granulation tissue?
Mucocele
What is this?
Mucocele
What is an example of a special type of mucocele?
Ranula
Where are ranulas found?
ONLY on sublingual gland (under tongue)
What is a sialolith?
Saliva gets calcified, turns into stone and blocks mucous from exiting duct
Where are sialoliths located?
Whartons duct (connects submandibular with sublingual gland)
What can pus coming out of an area indicate?
Somewhere nearby is a salivary stone
How can you see a sialolith?
Occlusal radiograph
What is the dx if pt comes in and says
“oh nooo a part of my mouth plopped out?”
Necrotizing sialometaplasia
What is necrotizing sialometaplasia characterized as?
Area of ulceration
What are symptoms of Sjogren’s?
Autoimmune
Dry mouth, dry eyes
Bilateral enlargement of parotid gland
How do you diagnose sjogrens?
Lower lip biopsy, should have lymphoid clusters
Certain amount of clusters = sjogrens
What is the most common benign salivary gland tumor?
Pleomorphic adenoma
Where are monomorphic adenomas found?
Upper lip
What gland is involved with papillary cystadenoma lymphomatosum?
Parotid gland (95%)
What are characteristics of Papillary cystadenoma lymphomatosum?
Parotid
Bilateral
Middle aged MALES
Smoking
What is the second most common benign salivary gland neoplasm?
Papillary cystadenoma lymphomatosum
What is oncocytoma?
Rare neoplasm
What is this?
Monomorphic adenoma
because on upper lip
What is the most common MALIGNANT salivary gland neoplasm?
Mucoepidermoid carcinoma
What is typically mistaken for a mucocele?
Mucoepidermoid carcinoma
DDx
Mucocele or mucoepidermoid carcinoma
What is the best hystologically recognized salivary gland tumor?
Adenoid cystic carcinoma
What typically histologically presents as swiss cheese?
Adenoid cystic carcinoma
What is a common MALIGNANT MINOR salivary gland neoplasm?
Polymorphous low-grade adenocarcinoma (PLGA)
- not on parotid, sublingual, or submand gland
What neoplasm favors the palate?
PLGA
- cause its a malignant MINOR neoplasm and lots of minor glands on palate
What is this?
PLGA
What neoplasm histologically appears with indian filing?
PLGA
Where is acinic cell carcinoma typically found?
Parotid gland (not exclusively)
What speed does acinic cell carcinoma grow?
Slow growing
What does acinic cell carcinoma histologically present as?
Serous acinar cells (looks like parotid gland)
What is this?
Acinic cell carcinoma
What is long standing pleomorphic adenoma if not treated?
A malignant transformation of pleomorphic adenoma -> benign mixed tumor (carcinoma ex-pleomorphic adenoma)
What are characteristics of carcinoma ex-pleomorphic adenoma?
Rapid growth
Older adults
Pain and ulceration
Histologically - benign mixed tumor
What are the benign salivary gland neoplasms? List them
- Pleomorphic adenoma
- Monomorphic adenoma
- Papillary Cystadenoma Lymphomatosum (Wharthin’s tumor)
- Oncocytoma
What are the malignant salivary gland neoplasms? List them
- Mucoepidermoid carcinoma
- Adenoid cystic carcinoma
- Polymorphous low-grade adenocarcinoma (PLGA)
- Acinic cell carcinoma
- Carcinoma ex- pleomorphic adenoma
- Adenocarcinoma NOS
What pathology is painless and slow growing?
Benign mixed tumor (plemorphic adenoma)
