Put these steps in order
a. Select open new tab icon
b. Select: Site map>Select Related Data>Sequence (up next)
c. Select: Sales Insights Settings>Sales Accelerator>Set Up
d. Select Done.
e. Choose content and layout to select the custom form you want to add the widget to.
f. Select: Form Designer> Select component, then from layout drag the column to the form
