what is an adverse event?
results in unintentional harm
- errors of commision:
- errors of omission?
- failure of doing something that should have been done.
- medication that was supposed to be given but was not.

action that was not completed, as intended
- patient had an allergy to PCN and was given it and had a reaction
- also called never event (event that should have never happened)

near miss event?
errors that could have harmed a patient but harm did not occur
-incorrect dosage was given
- patient was given the wrong med but no adverse reaction occurred
what is a sentinel event?
unexpected occurence involving death or serious injury
- patient falls are the #1 cause of events
what are the types of errors in diagnostics?
failure to conduct appropriate test/ failure to act on results
what are types of errors in treatment?
- errors in administration
- medication error
- failure to respond in the test result
what are types of errors that are preventative?
failure to provide, proper monitoring, or follow up
what type of errors that are the leading cause of harm?
