ultimate responsibilities with program lies with top managers but all employees are accountable for program compliance. walk the talk.
training, awareness, and competence (as part of h&s program)
employees must be competent in performing tasks to keep them safe. this is accomplished by identifying roles, analyzing and identifying training needs
consultation and communication (as part of h&s program)
programs are worthless if they aren’t followed or understood. management must establish effective consultation and communication processes that encourage participation and provide support
document and date control (as part of h&s program)
documents should be tracked and managed in a system that makes them available all employees and doesn’t allow unauthorized changes
ERP (as part of h&s program)
need ERP to be prepared for fires, explosions, gas releases, spills. crews should hold practice drills. should identify:
- types of emergencies
- alarm systems and equipment
- ert members and duties
- muster locations
- incident commend team
non conformance reporting program (as part of h&s program)
must have effective process/program for reporting and investigating incidents, accidents, and non conformance with safety requirements. program should include procedures for:
- reporting and investigating
- identify actions to prevent re occurance
- implement corrective/preventative actions
- validate effectiveness of corrective/preventative
safety program pyramid
nonconformance/inicidents/minor injuries/serious injuries/fatalities
performance monitoring, inspection, and auditing (as part of h&s program)
procedures must be in place to monitor these things to progress h&s safety goals
1st step in training program: analyze needs
matrix should be developed for each position to work their position safely
2nd step in training program: asses priorities
employees exposed to greatest risk in their jobs should be prioritized if resources/time is limited
3rd step in training program: design training
develop training packages that are relevant, asses pre and post knowledge and currency checks
4th step in training program: delivery
- classroom (teacher must be qualified, competent, and consistant)
- on the job (where hands on practical is required)
- correspondence
- internet based
5th step in training program: reinforce learnings
repeat and reinforce learnings. develop currency schedule for certain courses
6th step in training program: evaluate effectiveness
understanding of learnings must be checked. leading and lagging indicators are a quality check. procedure compliance is another check.
7th step in training program: continuous improvement
program must keep up with technology, regulatory, and plant changes
outline 6 step risk reduction process
classify work areas (steam water process)>identify hazards (HAZOP)> asses risk (likelihood+consequence)> develop risk reduction plan (establish priorities)> develop/implement controls (remove, procedures, ppe)> asses outcome (evaluate effectiveness)
Who should be involved in developing risk reduction plans? Why?
Plant personnel, allows focus to land on higher risk items and not get sidetracked with trivial matters.
Purpose of a safe work plan
identify hazards, asses risk, and apply controls for specific tasks. Should be joint effort between ops and maint who are familiar with work.

Macro-overall big picture philosophy of job
Micro-JSA, breaks job down into single tasks
