Confirmation
Matter: Chrism- an oil specially consecrated by the bishop.
Form: Action- anointing the recipient with chrism.
Words- The church’s prayer which accompanies the anointing (currently: “Be baptized with the gift of the Holy Spirit”)
Minister: A priest or bishop
Baptism
Matter: water
Form: Action- either immersing a person in water or pouring water on a person. Words- the Trinitarian baptismal formula based on Matthew 28:19.
Minister: Any human person who has the intention of baptizing as the church baptizes.
Eucharist
Matter: Bread and wine
Form: Action- The minister holding the bread and wine
Words- The words of institution.
Minister: A priest or bishop
Penance
Matter: the contribution of the penitent
Form: Action- performing the assigned penance
Words- An honest confession of sin by the penitent and the church’s prayer of absolution spoken by the minister.
Minister: a priest or bishop
Anointing of the sick
Matter: the oil of the infirm- a special oil consecrated by the bishop.
Form: Action- the minister anoints the recipient with the oil of infirm.
Words- the church’s prayer for this sacrament.

Matrimony
Matter: the consent of the spouses to the blessings of matrimony; fidelity, offspring.
Form: Action and Words- the vows of matrimony, pronounced publicly before the church in the presence of the church’s official witness.
Minister: A man and a woman who are free to marry.
Holy Orders
Matter: A baptized man who is free of canonical impediments to ordination.
Form: Action- the laying of hands upon the recipient by the minister.
Words- the church’s prayer for Holy Orders
Minister: a bishop
