RSI Flashcards
“Go to” intubation equipment sizes (blade and tube, male and female)
3 or 4 Mack or Miller
8-8.5 ETT for male
7.5-8 ETT for female
How does preoxygenation effect the patient?
Replaces the room air in the functional residual capacity with high concentration O2 and creates a surplus of O2 in the blood and tissue
About how long does it take for a healthy 70kg adult to desat from 100% to 90%?
about 8 minutes
How long does it take an OBESE 127kg to desat from 100% to 90%
About 3 minutes
Pretreatment drug mnemonic?
LOAD
Lidocaine
Opiod (usually Fentany)
Atropine
Defasciculating agent
Post intubation signs of a pneumothorax? (3)
difficulty bagging, ^ in PIP, decreased breath sounds
Should you bag a patient after induction and paralysis?
No, risk of gastric inflation and aspiration increases significantly
How much FiO2 does a NRB mask deliver?
70-75%
What is he single biggest anatomical obstacle to intubation?
The tongue
What is the BURP mnemonic and what is its purpose?
Pressure applied to the cricoid cartilage to better visualize the vocal cords, Backwards, Upwards, Rightward, Pressure
MOANS mnemonic and what is it designed to assess?
Mask seal (beards)
Obesity, obstruction
Age >55
No teeth
Stiff lungs - COPD, ARDS
All these components can lead to issues with bagging due to seal issues or bagging issues.
LEMON mnemonic and what does it evaluate?
Look, evaluate (3-3-2), mallampati score, obstruction, neck mobility. Used to evaluate difficulty of intubation.
What is the 3-3-2 score and what does it evaluate?
3 fingers in mouth, 3 fingers from chin to neck, 2 fingers from neck to adams apple. Used to evaluate anatomy and anticipate airway difficulty.
Minimum Cric supplies
Trouseeas dilator, tracheal hook, #11 blade, cuffed, non-fenestrated #4 trache tube
