Define RPD

A

Removable partial denture - a dental prosthesis designed to replace some (not all) of the teeth in a an arch of a partially edentulous patient. They distribute forces onto remaining soft and hard tissues in a easily placed/removable fashion

  1. Planning (the proper dx of which teeth to replace)
  2. Design of partial denture
  3. Pre-prosthetic treatment (periodontal tx)
  4. Tooth modification
  5. Framework fabrication
  6. Capture of supporting soft tissue
  7. Reconstructing the missing tissues
  8. Delivery and maintenance (you need tissue to remain healthy to avoid pulling teeth and loosening RPD)

Define philosophical profile

A

easy going, well adjusted patients involved in their oral care. They take responsibility for situation
most IDEAL patient who will tolerate well made prosthesis

Define exacting profile

A

Precise, perfectionist and demanding. These are difficult patients with high expectations who will require extra time. They must be warned of possible problems BEFORE they occur, and be given lots of attention (if you please them, they are great ambassadors)

Define hysterical profile

A

Apprehensive and complain without reason. Do not accept responsibility for their problems. * If attitudes cant be changed these patients will fail no matter what.*

Define indifferent profile

A

Unmotivated , unconcerned, uncooperative, ignore instructions, and show little concern for oral health. Poor prognosis unless attitudes change - RPD will end up causing harm

Systemic diseases that affect dental treament (8)

A
  1. DIABETES - poor tissue tone, abscesses, and reduced saliva. They have increased caries and difficult adaptation
  2. ARTHRITIS - Changes in TMJ (occlusion changes, difficulty determining centric relation)
  3. PAGET’S DISEASE - continued enlarment of maxillary tuberosities causing changes in fit and occlusions (more recalls required).
  4. ACROMEGALY - mandibular enlargement requires frequent examination for fit and function
  5. PARKINSON’S - rhythmic muscular contraction may cause difficulty to insert RPD and care for prosthesis (oral hygiene).
  6. PEMPHIGOUS VULGARIS - bullae in oral cavity, oral discomfort and dryness is erroneously linked to RPD but once diagnosed, no problems with RPD if maintained
  7. EPILEPSY - frequent seizures without warning make this CONTRAINDICATED, otherwise use radiopaque RPD and prevent drug induced gingival hyperplasia by using smooth polished borders on RPD
  8. CANCER - side effect of treatment can cause xerostomia, infections, mucosal irritation (difficulties arise with RPD)
