First Degree Atrioventricular (AV) Block

Rhythm: P-P and R-R is constant

Rate: Normal Limits 60-100 bpm

P: have the same configuration and shape, Each QRS complex will have a P wave, same number of P waves as QRS

PR: Greater than 0.20 sec.

QRS: WNL 0.06-0.10 seconds

Second Degree Atrioventricular (AV) Block, Mobitz I (Type I or Wenckebach)

Rhythm: P-P Regular. R-R Irregular.

Rate: Atrial rate WNL, Ventricuar is slower than atrial rate.

P: Normsl size nd upright wave, P for every QRS wave but there will be extra P waves

.PR: Vary measurement, progressively longer until QRS wave is dropped, PR will be short and then the cycle will begin again.

QRS: WNL 006-0.10 sec

Third Degree Atrioventricular (AV) Block (Complete)

Rhythm: P-P regular, R-R Regular but P-P and R-R have different measurements.

Rate: Atrial rate 60-100 bpm, Ventricular 20-40 bpm or 40-60 bpm, depedning upon pacemaker site

P: Normal size but may be buried within the QRS, either before or after, more P waves than QRS complexes, and not every P wave will have a QRS complex following.

PR: Variable due to the atrial and ventricular tissue depolarizing at different rates

QRS: Configuartion and duration will be the same but measurements may be either WNL or wide, depending on the area of blockage.

Accelerated Idioventricular Rhythm

Rhythm: P-P cannot be determined, R-R is regular

Rate: Atrial cannot be detremined due to abscence of atrial depolarization. Ventricular rate is 40-100 bpm

P: Usually absent

PR: Can’t determine without P waves

QRS: Wide and bizarre measures 0.12 sec or greater

Accelerated Junctional Rhythm

Rhythm: P-P and R-R are regular and similar intervals, P-P may be hard to measure becuase of location.

Rate: If P identifiable rate is 60-100 bpm, Ventricular rate 60-100 bpm

P: usually inverted and may precede, follow, or fall whithin the QRS complex. IT MAY NOT BE VISIBLE.

PR: If P wave before QRS then PR will measure less than 0.12 sec and will be constant. If P wave is not before the QRS, the PR interval cannot be determined. Because P wave is not associated with the next QRS complex.

Agonal Rhythm

Rhythm: P-P interval cannot be determined. R-R interval may or may not be regular.

Rate: Atrial rate connot be determined due to abscence of atrial depolarization, Ventricular rate is less than 20 bpm.

P: P wave is absent.

PR: cannot be measured becuase the P wave cannot be identified

QRS: Measures 0.12 sec or greater and is wide and bizarre

Rhtyhm: No waveforms = no P-P and R-R intervals

Rate: No atrial or ventricular rate are present.

P: No waves are present

PR: Unable to measure because no waveforms

QRS: Not measureable because no waveforms

Atrial Fibrillation

Rhythm: P-P is unable to be determined because of the fibrillatory waves or f waves. R-R interval is irregular.

Rate: Atrial rate not determined due to chaotic nature of waveforms, they are not contracting and pumping bl, Electrical impulse activity is 375-700

P: Cannot be identified, chaotic electrical activity, or “f” waves may be seen.

PR: Cannot be measured since P is not identifiable

QRS: WNL 0.06-0.10 sec and irregular

Atrial Flutter (Sawtooth)

Rhythm: P-P interval or flutter-flutter are regular, R-R is irregular but occasionaly it may regular.

Rate: atrial rate will be between 250-350 bpm

P: not seen and only flutter or (F) waves are present, more flutter waves than QRS

PR: No identifiable P wave exists so no measurements

QRS: WNL 0.06-0.10 sec

Bundle Branch Block

Rhythm: Regular or irregularity will depend on the underlying rhythm, sinus or atrial.

Rate: atrial and ventricular rates will depend on basic rhythm

P: will depend on the basic rhythm

PR: Normal 0.12-0.20 sec

QRS: Measurement 0.12 sec or greater, widening of the QRS duration indicates presence of BBB
