Aetiology

A
  • autoimmune disease : following strep ( present as sore throat , scarlet fever )
  • latent period : 2:4 weeks ( 4m for rheumatic chorea)

Pathogensis

A
  • genetic susbetability
  • cross reactivity:
    •molecular mimicry between human tissue antigen ,
    strep

• antibodies formed react with them instead

Pathology , incidence

A

Patho
1- proliferative lesion : aschoff nodules, heal by fibrosis

2- exudative lesion : effusion , heal without any residual change

Incidence
1- age : 5:15 ( recurrence is accomplished) ( no inf during infancy)

2- sex : equal ( chorea is more in f)
3- more common in developing countries ( prevelant)
4- status in egypt : public health problem, involves 5,1/1000 of school children

5- family history may by positive
6- the most common acquired hd in children

Jones criteria ‼️‼️‼️

A

MAJOR CRITERIA
1- polyarthritis ( mono arthritis, polyarthralgia are considered major in high risk populations)

2- carditis
3- chorea
4- erythema marginatum
5- sc nodules

MINOR CRITERIA
1- mono arthralgia ( ex if poly is major )
2- prolonged PR interval ( ex if carditis is major)
3- fever >_ 38 c
4- ESR >_ 30 , CRP >_ 3
5- passive rf

Evidence of recent strept inf
- throat swab or culture ( not reliable )
- ab
• aso
• antistreptokinase
• antihyalurindase
• antidnase
