Restorative options for dental injuries Flashcards
What are the options for fractured teeth?
Composite tips
Fragment re-attachment
How do you protect the dentine when placing a composite tip?
Place a GIC liner over the pulp
Why should a v shaped notch be placed in the fragment and the tooth be bevelled?
To increase the bonding surface area
What does a discoloured tooth signify?
Pulp canal obliteration
How can bleaching be carried out on non-vital teeth?
H2O2 (3%) and urea
Describe the process of non vital bleaching?
Etch pulp chamber
Sodium perborate
Repeated visits
Place non setting hypo cal inbetween visits
How common is bleaching relapse?
20% relapse in 3y
What are the bleaching brands available for vital teeth?
Opalescence
White and bright
Rembrandt
How should whitening trays be cut?
Relieve 0.5mm of labial surface of teeth
Act as reservoir of bleaching agent
Pre-op radiograph
Check shade and vitality
Check for sensitivity and discomfort after 2w
What are the rules on bleaching of under 18s?
Regulations and EU directive specifically state the product must not be used on patients under 18
If need for under 18s, must have detailed discussion with parents, ensure patients/parents are fully informed of the risks, benefits and alternatives and whether this can be delayed until after the child is 18
Document all consultations
Contact defence union for advice
What are the options for displaced teeth?
Repositioning
Autotransplantation
Replacement
- fixed
- removable
What causes ankylosed teeth?
Replacement resorption (significant damage to the PDL)
Bony remodelling
Loss of alveolar bone height
No effective tx
(ankylosis is progressive during growth)
How to preserve bone around these teeth?
Decoronation of residual root
Remove crown and root surface reduced to 2mm below proximal bone
Clot formation is important
Allows for aesthetic replacement (removable or fixed pros)
Want to encourage replacement resorption of remaining root to occur
