Third Line - moderate dose ICS plus LABA or leukotriene inhibitor (asthma with sinus disease or exercise induced)
Fourth Line - anti-IgE therapy (omalizumab)

Asthma Severity

Answer 2

Intermittent - symptomatic <2 days per week, <2x/month night time, non limiting, normal FEV1 between exacerbation
Persistent Mild - greater than 2/week but not daily, 3-4x at night, minor limitation, FEV1 >80%
Persistent Moderate - daily symptoms, 1x/week at night, mild limitations, FEV1 >60 <80%
Persistent Severe - multiple daily, 7x week at night, extreme limitation, FEV1 <60%
