Respiratory System Overview Flashcards

1
Q

What are the 2 main components of the respiratory system?

A
  1. Ventilation
    - airway patency
    - active muscles
  2. Gas exchange
    - adequate number of alveoli
    - no fibrosis of alveolar wall

Q

What muscles can act as accessory muscles for ventilation?

A

Pectoralis (major and minor)
Sternomastoid
Note: it is necessary to fix the position of arms for this

Q

What is the limiting factor affecting gas exchange when ventilation is adequate?

A

Ability of alveoli to get O2 into the blood and CO2 out of the blood

Q

Name 3 reasons that effect the ability of alveoli to function properly

A
  1. thickening of alveolar wall
    - makes diffusion harder
  2. Inadequate number of alveoli (emphysema)
  3. V-Q mismatch
    - mismatch between where air goes into the lungs and where blood goes into the lungs

Q

List 5 common respiratory symptoms

A
  1. cough
    - dry, sputum (colour?), blood
  2. Wheeze - expiratory noise
  3. Stridor - inspiratory noise
  4. Dyspnoea (distress on effort)

Q

List 5 common respiratory signs to look for

A
  1. Chest movement with respiration
    - should be equal on both sides
  2. Rate of respiration
    - normally 12-15 breaths/minute at rest
  3. Air entry
    - should be symmetrical, check if it is reduced in any areas
  4. Vocal resonance
    - should be poor in a normal patient
  5. Percussion note
    - resonant: suggests air
    - dull: suggests solid or liquid
17
Q

What respiratory investigations should be carried out in patients with respiratory problems? (5 main ones)

A
  1. sputum examination
  2. CXR - chest radiograph
  3. CT scan
  4. Pulmonary function - spirometry
    - PEFR - máximum flow rate
    - FEV1 - forces expiratory volume
    - FEV1/VC - measure of respiratory function
  5. bronchoscopy
  6. VQ scran - ventilation/perfusion mismatch
