Respiratory Emergencies - Adams Flashcards
Remember airway management does not equal ______?
endotracheal intubation
If labored breathing consider helping assist their breathing by doing what?
Utilizing continuous positive airway pressure (CPAP) or bilevel positive airway pressure (BiPAP)
What does BiPAP reduce?
BiPAP reduces the work of breathing out
What is High flow humidified air used for?
- Use in bronchiolitis, asthma, COPD (loosens up the junk)
- Creates a reservoir of O2 and moisture
What do you use prior to intubation?
assist oxygenation by using a bag-valve mask
leave dentures in, need to get a good seal on the face
What are some injuries or illnesses that may demand airway management ?
- Trauma (facial/neck injuries, flail chest)
- Illnesses/Hypoxia (COPD, edema, pneumoina, etc.)
- Mechanical (compression, obstruction, foreign body)
- Burns
When should you intervene if a patient is unable to protect their airway?
-Decreased level of consciousness (LOC) => GCS
What are primary reasons NOT to intubate?
- adequate airway protection is present
- oxygenations by other less invasive means can be done
- the patient is “Do Not Intubate” (DNI) => Do not resuscitate” (DNR) does not mean “do not intubate” (DNI)
What are the back up options to intubation?
Combitube
King Airway
Laryngeal mask airway (LMA)
What patients are potentially difficult intubations?
Short neck Prominent upper incisors Receding mandible Limited jaw opening Limited cervical spine mobility
What is the only depolarizing neuromuscular blocker that is an ultrashort-acting skeletal muscle relaxant that combines with cholinergic receptors at the motor endplate to produce flaccid paralysis?
Succinylcholine
What are the adverse side effects of Succinylcholine?
-Bradycardia. Treat with atropine -Increased: Intragastric pressure (IGP) Intracranial pressure (ICP) Intraocular pressure (IOP) Serum potassium levels go up Temperature
What are the two Nondepolarizing Neuromuscular Blockers used for intubation?
Vecuronium
Rocuronium
What NON-barbiturate sedative used in intubation is a mild sympathomimetic, is useful in trauma and hypotensive states, and has bronchodilatory property is useful in asthmatics?
Ketamine
What Benzodiazepine is used in intubation is a short-acting CNS depressant, which also causes a lack of recall?
Midazolam
