Salbutamol
(SABA) Asthma and COPD
Short acting, inhaled (neb), IV, oral
Binds to beta(2)-receptors in the lungs relaxing bronchial smooth muscles.
Increases cAMP production by activating adenylate cyclase
Salmeterol (formoterol)
(LABA) Asthma in pt req. long dose bronchodilator therapy on ICS (not PRN) COPD: persistent symptoms despite therapy (ICS/LABA combination FEV1 < 50%) - seretide
Long acting, inhaled (neb)
Long-acting beta-adrenoceptor agonist
Tiotropium, Aclidinium bromide
(LAMA) Stable COPD (symptoms despite SABA), asthma
Long acting (24 hour), inhaled

Long-acting, antimuscarinic/anticholinergic
Affinity to M1 - M5, inhibition of M3 on smooth bronchial muscle causes bronchodilation

PO in persistent COPD/asthma
IV in asthma emergencies
Phosphodiesterase inhibitor
Adverse effects of Bronchodilators
Tachycardia
Nervousness, Irritability, Tremor
Oral > neb: tachyarrhythmias/Angina
Usually dose related
Perfenidone
Idiopathic pulmonary fibrosis
Anti inflammatory/ Anti Fibrotic
Reduces fibroblast proliferation, reduces collagen production

HAP, gram -ve (extended spectrum b lactam)
Doxycycline
Tetracycline, inhibits protein synthesis, gram +ve and -ve
Useful for atypical infections eg. mycoplasma
Oral only
Side effects - GI Upset, staining teeth, lupus, allergy, photosensitivity
Beclometasone (intranasal)
Allergic rhinitis
Pseudoephedrine
Allergic rhinitis, alpha agonist, anticongestant, problems with abuse and combinations with MAOI
Cetirizine, Chlorpheniramine
Antihistamines H1 antagonists
Se: drowsiness, dry mouth, dry eyes, confusion
DOT and VOT
Directly/video observed treatment eg. for those unable to adhere with six months of treatment
Prednisolone/Azathioprine/MMF
Idiopathic interstitial pneumonia
