Zileuton

A

Leukotriene synthesis inhibitor

Inhibiting the enzyme, 5-lipoxygenase.

treatment of mild and moderate persistent asthma.

Contraindications - previous HSR to drug, history of hepatic disease/ elevations in hepatic enzymes.

Zileuton interferes with the metabolism of theophylline, warfarin, and propranolol, neuropsychiatric events and hepatic injury.

Theophylline / Aminophylline

A

Methylxantine

antagonist of adenosine receptors.

Theophylline -non-selective inhibitor of phosphodiesterase 4 (PDE4 - degrades cAMP)

Theophylline is used for the prophylactic treatment of mild and moderate persistent asthma. Aminophylline is given intravenously for the treatment of status asthmaticus.

Contraindications and Cautions – Potential cardiac arrhythmias and seizures , the use of theophylline is contraindicated in patients with heart disease and with a history of seizures.

theophylline is contraindicated in patients with liver disease. In addition, cimetidine, a histamine H2-receptor antagonist, increases the plasma levels of theophylline when co-administered.

Side Effects – Side effects to theophylline include nausea, vomiting, cardiac arrhythmias, hypotension, cardiac arrest, and seizures.
