key words, labs and managment
kids 3mo-5yr in fall winter
days 1-3: runny nose, low grade fever, then hoarseness, barking cough, stridor + respiratory distress
steeple sign= narrowed ariway
75% parainfluenza virus
mild→ oral steroids 1mg/kg/day for 3 days
moderate (stridor at rest) /severe→ admit to ED, racemic epi via nebulizer, 02 prn, observe 3hrs (rebound RD)
epiglotitis

acute onset fever
difficulty speaking, swallowing
**do NOT manipulate oral cavity if drooling and RD (can obstruct airway)
NO cough
toxic appearing
thumb sign if x-ray done (usually not needed)
contacts receive rifampin prophalaxis
MC causitive agents of epiglotitis
was H. influenza pre-vaccine
Hib vaccine schedule→ 3 shots by 1 yr, 4 shots by 4yr
Now see staph and strep causing
