Goals of treatment in COPD
Salbutamol improves lung recoil, tiotropium decreases gas trapping, steroids decreases exam and hospital admissions, Pulmonary rehab improves QoL
NSCLC TNM staging
T1 < 3 cm without invasion
T2 > 3cm OR local invasion
T3 Tumour of any size with invasion into mediastinum
N - 0 no nodes, N1 metastases into ipsilateral hilarious and /or ipsilateral peribronchial nodes, N2 mets to hilarious / subcarinal nodes. N3 mets to contralateral nodes
M0 - no mets, M1 distant mets
Treatment NSCLC
Chemotherapy for stage 3 and above OR any T with N2 (Mediastinal or subcarinal involvement). Surgery only for localised disease
Paraneoplastic Neurological symptoms of lung cancer
Horners, UL signs ? pancoast, Recurrent laryngeal nerve palsy, diaphragmatic paralysis.
Paraneoplastic Neuro Sx -
Lambert Eaton W/ small cell (voltage gated Ca channel Abs)
Peripheral neuropathy, cerebellar degeneration, dematomyosotis, Polymyositis, acute transverse myelopathy
