Process of taking oxygen into the body and expelling carbon dioxide from the body Is one inspiration and one expiration Do not let the patient know that you are counting respirations (if the patient knows what you are doing, they may not breathe normally)

Rhythm – are the respirations regular or irregular?
Dyspnea – is breathing difficult?
Apnea – has breathing stopped?
Tachypnea – high resp rate - > 25 resps/min
Bradypnea – low resp rate - < 10 resps/min
Cheynes-stokes – Are there periods of labored
respirations followed by apnea?
Rales – can you hear bubbling or rattling sounds
caused by mucus in the air passages?
Wheezing – difficult breathing with a high
pitched whistling or sighing sound
Cyanosis – bluish discoloration of the skin, lips,
and/or nail beds due to decreased O2 and
increased CO2 in the bloodstream
