Native valve endocarditis (sub-acute)
Amoxicillin IV 2g 4 hourly + Gentamicin 1mg/kg (NAG)
Native valve endocarditis (acute sepsis) or IVDU
Flucloxicillin IV 2g 6 hourly
Prosthetic valve endocarditis or MRSA
Vancomycin IV + Rifampicin PO 600mg + Gentamicin IV 1mg/kg (VGR-ous treatment)
Non-severe C. Difficile
Metronidazole PO 400mg
Severe C. Difficile
Vancomycin 125mg qd PO (only time you give it orally) +/- IV Metronidazole
Peritonitis/Biliary tract/Intra-abdominal
IV Amoxicillin + Metronidazole + Gentamicin
(then step down to PO Co-trimoxazole + Metronidazole)
If penicillin allergic: IV Vancomycin + Metronidazole + Gentamicin
Gastroenteritis
No antibiotic treatment required unless systemic
Systemic campylobacter
Erythromycin or Ciprafloxin for 5 days
Systemic salmonella
Ciprafloxin for 5 days
Typhoid/paratyphoid fever
Depending on sensitivities. IV Ceftriaxone if unstable
Any unknown infection
IV Amoxicillin + Metronidazole + Gentamicin
if allergic IV Vancomycin + Metronidazole + Gentamicin
Giardia
Oral Metronidazole
Threadworms
Oral Mebendazole
4 C’s antibiotics associated with C. Difficile
Clindomycin, Cephalosporin, Co-amoxiclav, Ciprafloxin
Amoebiasis
Metronidazole and remove from lumen with diloxinide furate or puromycin
