Resp Flashcards
What causes low blood pressure in someone with a PE?
Right heart strain
Management of PE if haemodynamic instability
Thrombolysis - alteplase
Stable PE without haemodynamic instability
- anticoagulation LMWH (quicker onset of action)
or also warfarin/DOACS
THhrombolysis
Thrombectomy
What is pulmonary hypertension
Pulmonary artery systolic pressure over 25mmHg
Pulmonary artery hypertension
Chronic thromboembolic pulmonary hypertension - VQ scan
PHLHD
What are the types of pulmonary hypertension?
Group 1 - PAH, idiopathic, hereditary, drug/toxins. CTD
Group 2 - secondary to left heart failure
Group 3 - secondary to lung disease and hypoxia
Group 4 - chronic thromboembolic pulmonary hypertension
Group 5 - Secondary to conditions with multifactorial mechanisms causing PH
Pulmonary wedge pressure
Mx of pulmonary hypertension groups
Group 1 - CCB
Group 2 - diuretics/HF mx
Group 3 - treat lund dsiaese with LTOT or CPAP
Group 4- anticoag, pulmonary endarterectomy, vasodilator thearpy
Asthma episodic symptoms (four)
Breathlessness
Wheeze
Chest tightness
Cough
Diagnosis of asthma
DIurnal variation of peak flow
HIstory of atopy
Reversibility with bronchodilator (FEV1 ncrease of 12% or 200mls)
Raised FeNO
Raised eosinophils
Raised total IgE
Positive provocation testing
Acute mx of asthma
O2
Neb salb
Neb ipratropium bromide
IV Hydrocortisone
Nbx
IV mag sulf
IV aminophylline
Inability to complete sentences - severe
Silent chest - fatal asthma
If on a repeat ABG in an asthma attack,
If co2 rises and po2 still low,
