How is estrogen metabolized?
binds to albumin and sex hormone binding globulin in serum, undergoes metabolism in liver and kidney excretion
What are the Estrogen agents?
Conjugated estrogens, esterified estrogens, estradiol, ethinyl estradiol, chlorotrianisene
What is estrogen used for
oral contraceptive, postmenopausal hormone replacement, osteoporosis, hypogonadism in girls 11-13, palliative treatment of incurable prostate cancer
What are primary AE of estrogen tx?
hypertension (estrogens increase angiotensinogen), gallbladder disease, thromboembolytic complications (estrogen increases synthesis of clotting factors), melasma– skin darkening
What are the SERMs, Selective estrogen receptor modulators?
Clomiphene, Tamoxifen, Raloxifene
How do the SERMs work?
interfere with negative feedback of estrogens on hypothalmaus, leading to increased GnRH and gonadotropin release, stimulating ovulation
What is Clomiphene used for?
treat infertility but has high risk of multiple births
What is Tamoxifen used for?
palliative treatment of advanced breast cancer, decreases DVT but increased N/V/hot flashes
What is Raloxifene used for?
prevention of osteoporosis in post menopausal women
What are the Aromatase Inhibitors?
Anastrozole, Letrozole
What is Fulvestrant used for?
pure estrogen receptor antagonist. Used to treat tamoxifen resistant cancer.
What are the aromatase inhibitors used for
adjuvant tx of postmenopausal women with hormone receptor positive breast cancer
How does the progestins work?
high levels inhibit gonadotropin release and thus ovulation, thickens cervical mucus, creates a thin atrophic endometrium that prevents implantation
What are synthetic progesterone agents
Hydroxyprogesterone, medroxyprogesterone, Norethynodrel, Norethindrone, Norgestrel
Which synthetic progesterone agents have androgenic activity
Nogestrel, Norethindrone
